儿童患者中分离的解甘露醇罗尔斯顿菌的流行病学及耐药分析

摘要 目的了解浙江大学医学院附属儿童医院临床分离的解甘露醇罗尔斯顿菌的临床资料及对抗菌药物的敏感性,为临床合理使用抗菌药物提供依据。方法收集2015年1月1日—2023年12月31日本院儿童患者临床分离的52株非重复解甘露醇罗尔斯顿菌,应用WHONET5.6软件进行统计学分析。结果解甘露醇罗尔斯顿菌感染科室主要来自ICU(48.1%),感染类型以呼吸道感染为主(67.3%),人群多见于婴儿(55.8%)。解甘露醇罗尔斯顿菌敏感率最高的抗菌药物为多西环素(100%),其次为环丙沙星和左氧氟沙星(98%);耐药率最高的抗菌药物为美罗培南和氨曲南(100%),其次为阿米卡星和妥布霉素(87.5%,85.7%)。解甘露醇罗尔斯顿菌对亚胺培南的耐药率(24.5%)远低于美罗培南(100%)。结论儿童患者中解甘露醇罗尔斯顿菌多引起免疫力低下人群的呼吸道感染,且多为多重耐药菌,临床应根据药敏结果合理使用抗菌药物。 Objective To examine the clinical characteristics and antibiotic susceptibility profiles of Ralstonia mannitolilytica strains isolated from pediatric patients at Children's Hospital,Zhejiang University School of Medicine,and provide laboratory data to inform the judicious use of antimicrobial agents in clinical settings.Methods A total of 52 Ralstonia mannitolilytica strains were collected from pediatric patients from January 2015 to December 2023.The data were subjected to statistical analysis using WHONET 5.6 software.Results The study found that the majority of Ralstonia mannitolilytica infections originated from the Intensive Care Unit(ICU),accounting for 48.1%of cases.The predominant type of infection was respiratory tract infection,representing 67.3%of cases,and the affected population was primarily infants,comprising 55.8%of the cases.Antibiotic susceptibility testing revealed that doxycycline exhibited a 100%sensitivity rate,while ciprofloxacin and levofloxacin demonstrated a sensitivity rate of 98%.Conversely,the highest resistance rates were observed for meropenem and aztreonam,both at 100%,followed by resistance rates for amikacin at 87.5%and tobramycin at 85.7%.The resistance rate for imipenem(24.5%)was significantly lower than that for meropenem(100%).Conclusion Ralstonia mannitololytica frequently induces respiratory tract infections in pediatric patients with compromised immune systems.The majority of Ralstonia mannitololytica strains exhibit multidrug resistance.It is imperative for clinicians to select antibiotics judiciously based on antimicrobial susceptibility testing results.
