摘要
目的分析预见性护理结合失效模式与效应分析(failure mode and effect analysis,FMEA)模式预防2型糖尿病伴心力衰竭患者呼吸机相关性肺炎(ventilator associated pneumonia,VAP)的效果。方法回顾性选取2022年2月—2024年2月福州市第二总医院收治的104例2型糖尿病伴心力衰竭且使用呼吸机辅助治疗患者的临床资料,根据护理方法不同分为两组,各52例,对照组护理方法为常规护理,观察组在常规护理基础上,实施预见性护理结合FEMA模式。比较两组的血糖(空腹血糖、餐后2 h血糖)水平、VAP发生率以及心理状态。结果护理后,观察组患者的空腹血糖、餐后2 h血糖水平较对照组均更低,差异均有统计学意义(P均<0.05)。观察组VAP发生率较对照组更低,差异有统计学意义(P<0.05)。护理后,观察组心理状态评分优于对照组,差异均有统计学意义(P均<0.05)。结论对于2型糖尿病伴心力衰竭且使用呼吸机辅助治疗的患者,预见性护理结合FEMA模式可降低VAP的发生风险,缓解负面情绪,增强血糖水平控制效果。
Objective To analyze the effect of predictive nursing combined with failure mode and effect analysis(FMEA)mode in preventing ventilator associated pneumonia(VAP)in patients with type 2 diabetes mellitus complicated with heart failure.Methods The clinical data of 104 patients with type 2 diabetes mellitus complicated with heart failure who were treated with ventilator assisted therapy in the Fuzhou Second General Hospital from February 2022 to February 2024 were retrospectively selected.According to different nursing methods,they were divided into two groups,with 52 cases in each group.The control group was treated with routine nursing,and the observation group was treated with predictive nursing combined with FEMA mode on the basis of routine nursing.The blood glucose(fasting blood glucose,2-hour postprandial blood glucose)levels,VAP incidence and psychological status of the two groups were compared.Results After nursing,the levels of fasting blood glucose and 2-hour postprandial blood glucose in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The incidence of VAP in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).After nursing,the psychological state scores of the observation group were better than those of the control group,and the differences were statistically significant(all P<0.05).Conclusion For patients with type 2 diabetes mellitus complicated with heart failure and treated with ventilator assisted therapy,predictive nursing combined with FEMA model can reduce the risk of VAP,alleviate negative emotions,and enhance the effect of blood glucose level control.
