摘要
目的探究六味地黄汤加减中药泡洗联合西药治疗糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的临床效果。方法回顾性选取2024年1月—2025年1月景泰县中医医院收治的80例DPN患者的临床资料,根据不同治疗方法分为两组,其中采用西药治疗的40例患者为对照组,联合六味地黄汤加减中药泡洗的40例患者为观察组。比较两组中医证候积分、神经传导速度、炎症反应、临床疗效、不良反应。结果观察组治疗后肢体麻木疼痛、皮肤干燥脱屑、感觉减退或消失评分、肿瘤坏死因子-α、白细胞介素-6、内皮素-1、超敏C反应蛋白水平均低于对照组,正中及腓总神经的运动神经传导速度、感觉神经传导速度均高于对照组,差异均有统计学意义(P均<0.05)。观察组治疗总有效率较对照组高,差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论DPN患者采用六味地黄汤加减中药泡洗、西药联合治疗可减轻临床症状及炎症反应,提高神经传导速度,且安全性高。
Objective To explore the clinical effect of modified Liuwei Dihuang Decoction traditional Chinese medicine soaking and washing combined with western medicine in the treatment of diabetic peripheral neuropathy(DPN).Methods The clinical data of 80 patients with DPN admitted to Jingtai County Traditional Chinese Medicine Hospital from January 2024 to January 2025 were retrospectively selected and divided into two groups according to different treatment methods.Among them,40 patients treated with Western medicine were the control group,and 40 patients treated with modified Liuwei Dihuang Decoction combined with traditional Chinese medicine soaking and washing were the observation group.The traditional Chinese medicine syndrome scores,nerve conduction velocity,inflammatory response,clinical efficacy and adverse reactions were compared between the two groups.Results After treatment,the scores of limb numbness and pain,skin dryness and desquamation,and sensory reduction or loss,as well as the levels of tumor necrosis factor-α,interleukin-6,endothelin-1,and high-sensitivity C-reactive protein in the observation group were all lower than those in the control group,the motor nerve conduction velocity and sensory nerve conduction velocity of the median and common peroneal nerves in the observation group were both higher than those in the control group,and the differences were all statistically significant(all P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Liuwei Dihuang Decoction traditional Chinese medicine soaking and washing combined with Western medicine can reduce clinical symptoms and inflammatory response,improve nerve conduction velocity,and has high safety in patients with DPN.
