摘要
目的探讨全自动生化分析仪联合尿沉渣分析仪在早期糖尿病肾病(diabetic kidney disease,DKD)诊断中的临床价值与诊断效能。方法方便选取2021年3月—2024年12月平和县医院收治的160例疑似早期DKD的2型糖尿病患者,均接受全自动生化分析仪检测尿白蛋白/肌酐比值(urine albumin-to-creatinine ratio,UACR),并采用全自动尿沉渣分析仪检测尿液肾小管上皮细胞(renal tubular epithelial cells,RTECs)、红细胞(red blood cells,RBCs)及白细胞(white blood cells,WBCs)计数。以肾穿刺活检作为DKD诊断金标准,对比单用生化分析仪、尿沉渣分析仪检测及二者联合检测在DKD诊断中的诊断效能。结果早期DKD患者尿RBCs及WBCs水平与非DKD患者对比,差异均无统计学意义(P均>0.05);早期DKD患者UACR及尿RTECs水平高于非DKD患者,估算肾小球滤过率水平低于非DKD患者,差异均有统计学意义(P均<0.05)。联合检测检出阳性96例,阴性64例。联合检测的准确度、灵敏度、特异度及阴性预测值及阳性预测值均高于两种检测方法单独检测,差异均有统计学意义(P均<0.05)。结论全自动生化分析仪与尿沉渣分析仪联用可进一步提升早期DKD诊断的灵敏度、特异度及准确度,具有较高的DKD早期筛查与诊断价值。
Objective To explore the clinical value and diagnostic efficacy of automatic biochemical analyzer combined with urinary sediment analyzer in the diagnosis of early diabetic kidney disease(DKD).Methods From March 2021 to December 2024,160 patients with type 2 diabetes mellitus suspected of early DKD were conveniently selected from Pinghe County Hospital.The urine albumin-to-creatinine ratio(UACR)was detected by automatic biochemical analyzer.Urine renal tubular epithelial cells(RTECs),red blood cells(RBCs)and white blood cells(WBCs)were detected by automatic urine sediment analyzer.The gold standard for early DKD diagnosis was renal biopsy.The diagnostic efficacy of biochemical analyzer,urinary sediment analyzer and combined detection of the two in the diagnosis of DKD was compared.Results There was no significant difference in urine RBCs and WBCs levels between early DKD patients and non-DKD patients(both P>0.05).The levels of UACR and urinary RTECs in early DKD patients were higher than those in non-DKD patients,and the level of estimated glomerular filtration rate was lower than that in non-DKD patients,the differences were statistically significant(all P<0.05).Combined detection detected 96 positive cases and 64 negative cases.The accuracy,sensitivity,specificity,negative predictive value and positive predictive value of combined detection were higher than those of the two detection methods alone,and the differences were statistically significant(all P<0.05).Conclusion The combination of automatic biochemical analyzer and urinary sediment analyzer can further improve the sensitivity,specificity and accuracy of early DKD diagnosis,and has high value in early screening and diagnosis of DKD.
