摘要
目的分析糖化血红蛋白、空腹血糖、外周血C肽对2型糖尿病患者微血管并发症的预测价值。方法随机选取2025年2—6月于合肥市红花岗区人民医院体检的100例糖尿病高危患者为研究对象,均行糖化血红蛋白、空腹血糖、C肽检查,依据诊断结果分为2型糖尿病组(n=81)与非糖尿病组(n=19),将2型糖尿病组按照是否合并微血管并发症分成糖尿病组(单纯糖尿病,n=61)和合并组(2型糖尿病患者微血管并发症,n=20),比较两组患者血清指标水平,经Spearman相关系数分析各项指标与疾病的相关性,采用受试者操作特征(receiver operating characteristic,ROC)曲线评估联合指标对微血管并发症的预测价值。结果与非糖尿病组比较,2型糖尿病组的空腹血糖、糖化血红蛋白水平更高,差异均有统计学意义(P均<0.05)。合并组患者空腹血糖、糖化血红蛋白水平均高于糖尿病组患者,差异均有统计学意义(P均<0.05)。Spearman相关性分析显示,空腹血糖、糖化血红蛋白水平与2型糖尿病患者微血管并发症呈正相关(r=0.503,0.757;P均<0.05)。ROC曲线分析显示联合检测的灵敏度和特异度更高。结论糖化血红蛋白、空腹血糖均可预测2型糖尿病患者微血管并发症发生风险,联合检测可进一步提高预测价值。
Objective To analyze the predictive value of glycated hemoglobin A1c,fasting blood glucose and peripheral blood C-peptide for microvascular complications in patients with type 2 diabetes mellitus.Methods A total of 100 high-risk diabetic patients who underwent physical examinations at Zunyi Honghuagang District People's Hospital from February to June 2025 were randomly selected as the research subjects.All of them underwent tests for glycated hemoglobin,fasting blood glucose,and C-peptide.Based on the diagnostic results,they were divided into the type 2 diabetes group(n=81)and the non-diabetes group(n=19),the type 2 diabetes group was divided into the diabetes group(simple diabetes,n=61)and combined group(the type 2 diabetes patients with microvascular complications,n=20)based on whether they had microvascular complications.The levels of seum markers in the two groups were compared,and the correlations between each index and the disease were analyzed by Spearman correlation coefficient,receiver operating characteristic(ROC)curve was used to evaluate the predictive value of combined indicators for microvascular complications.Results Compared with the non-diabetes group,the fasting blood glucose and glycated hemoglobin A1c levels in the type 2 diabetes group were higher,and the differences were statistically significant(both P<0.05).The levels of fasting blood glucose and glycated hemoglobin A1c in the combined group were higher than those in the diabetes group,and the differences were statistically significant(both P<0.05).Spearman correlation analysis showed that fasting blood glucose and glycated hemoglobin A1c levels were positively correlated with micro-vascular complications in patients with type 2 diabetes(r=0.503,0.757;both P<0.05).ROC curve analysis showed that the combined detection had higher sensitivity and specificity.Conclusion Both glycated hemoglobin A1c and fast-ing blood glucose can predict the risk of microvascular complications in patients with type 2 diabetes.The combined detection of peripheral blood C-peptide levels can further increase the predictive value.
