摘要
目的分析艾司氯胺酮联合丙泊酚静脉麻醉对糖尿病合并子宫肌瘤宫腔镜手术患者血糖水平的影响。方法便利选取武夷山市立医院2022年8月—2024年12月收治的糖尿病合并子宫肌瘤患者98例,以差异性麻醉药物分为两组,各49例,对照组采用枸橼酸舒芬太尼注射液+丙泊酚乳状注射液麻醉,研究组采用盐酸艾司氯胺酮+丙泊酚乳状注射液麻醉,对比两组认知功能、恢复质量、血糖水平、疼痛程度。结果相比于对照组术后1 d、2 d,研究组恢复质量评分更低,差异均有统计学意义(P均<0.05)。术后2 h,对照组空腹血糖水平较术前升高,研究组血糖水平低于对照组,差异均有统计学意义(P均<0.05)。相比于对照组术后1 h与术后2 h,研究组疼痛程度评分均更低,差异均有统计学意义(P均<0.05)。两组术后2 h认知功能评分均低于术前,差异均有统计学意义(P均<0.05);但术前及术后2 h组间比较,差异均无统计学意义(P均>0.05)。结论糖尿病合并子宫肌瘤宫腔镜手术患者术中使用艾司氯胺酮联合丙泊酚静脉麻醉,对血糖水平及认知功能影响较小,能够提高患者恢复质量,术后患者疼痛感较低。
Objective To analyze the effect of esketamine combined with propofol intravenous anesthesia on blood glucose levels in patients with diabetes mellitus complicated with uterine fibroids undergoing hysteroscopic surgery.Methods Ninety-eight diabetic patients with concurrent uterine fibroids who were admitted to Wuyishan Municipal Hospital between August 2022 and December 2024 were selected and randomly assigned to two groups of 49 individuals each,according to the type of anesthetic agent administered.The control group was anesthetized with sufentanil citrate injection+propofol emulsion injection,while the study group was anesthetized with esketamine hydrochloride+propofol emulsion injection.Compared the cognitive functions,recovery quality,blood glucose levels and pain degrees of the two groups.Results Compared with the control group 1 day and 2 days after the operation,the recovery quality score of the study group were lower,and the differences were statistically significant(both P<0.05).2 hours after the operation,the fasting blood glucose level in the control group was higher than that before the operation,while that in the study group was lower than that in the control group,the differences were statistically significant(both P<0.05).Compared with the control group at 1 hour and 2 hours after the operation,the pain degree scores of the study group were lower,and the differences were statistically significant(both P<0.05).The cognitive function scores of both groups 2 hours after the operation were lower than those before the operation,and the differences were statistically significant(both P<0.05);however,there were no statistically significant differences between the groups before the opera-tion and 2 hours after the operation(both P>0.05).Conclusion The application of esketamine combined with propofol intravenous anesthesia in patients with diabetes mellitus complicated with uterine fibroids undergoing hysteroscopic surgery has little effect on blood glucose levels and cognitive function,which can improve the recovery quality of pa-tients and reduce postoperative pain.
