摘要
目的了解四川省人间布鲁氏菌病(布病)流行特征和空间聚集性,为精准防控布病提供科学依据。方法收集2020—2024年布病报卡数据,采用描述性流行病学方法分析流行特征,用ArcGis 10.6软件进行空间自相关分析。结果2020—2024年累计报告发病1453例,年均发病率0.347/10万,发病数、发病率连续五年上升。波及100%市(州)、78.7%区(县)。74.1%病例2—8月发病,男女比为2.5∶1,30~70岁组占86.9%,62.3%职业为农民。布病有4个高发地区:凉山州、成都平原、川南片区(泸州、自贡、内江)、川北片区(巴中、达州、广元)。2021—2024年布病发病存在空间正自相关(全局Moran's I值0.188~0.311,均P<0.001);凉山州连续四年存在高-高聚集区(热点),且热点地区比较固定,主要在西昌及其周边的冕宁县、喜德县、盐源县等地区;低—低聚集区(冷点)主要分布在川西地区的牧区。结论四川省布病疫情呈上升趋势,疫区范围不断扩大,呈季节性、职业性特征。建议西昌及周边县热点地区政府开展畜间布病免疫接种,热点外地区及时发现和处置人畜间疫情。
This study was aimed at understandingthe epidemic characteristics and spatial clustering of human brucellosis in Sichuan,to province a scientific basis for precise prevention and control of brucellosis,which is caused by bacteria in the genus Brucella.We collected brucellosis report card data from 2020 to 2024,then used descriptive epidemiological methods to analyze the epidemic characteristics and ArcGis 10.6 software for spatial autocor analysis.A total of 1453 cases were reported in 2020-2024,and the average annual incidence rate was 0.347/100000.The number of cases and incidence rate rose for five consecutive years.Moreover.100%of cities(states)and 78.7%of districts(counties)were affected;74.1%of cases occurred from February to August;the male-to-female ratio was 2.5∶1;86.9%of the cases were in the age group of 30-70 years;and 62.3%of the cases were in farmers.Four epidemic areas of brucellosis were identified:Liangshan State,Chengdu Plain,southern Sichuan(Luzhou,Zigong,Neijang Cities),and northern Sichuan(Bazhong,Dazhou,and Guangyuan Cities).From 2021 to 2024,a spatial positive auto-correlation of brucellosis incidence(global Moran's I of 0.188-0.311,all P<0.001)was observed.Liangshan State was a high-high cluster area(hotspot)for four consecutive years.The hotspot areas were relatively fixed and were mainly in Xichang and its surrounding counties,such as Mianning,Xide,and Yanyuan.The low cluster areas(cold spots)were distributed primarily in the pastoral area of western Sichuan.The reported incidence of brucellosis in Sichuan Province showed an upward trend and spread of epidemic areas,with seasonal and occupational characteristics.We suggest that the governments of hotspot areas,such as Xichang and surrounding counties,undertake animal brucellosis vaccination,as well as efforts for timely detection and disposal of human and animal epidemics in the areas outside the hotspot.
