消化内镜运用全自动软式内镜清洗消毒机清洗消毒的质量研究

摘要 目的对比分析自动软式内镜清洗消毒机与手工清洗软式内镜的清洗消毒效果,希望能进一步提升内镜的清洗质量,保证医患安全。方法选取某院消毒供应中心2021年1月—2023年6月软式内镜322条次,按照组间基本特征具有可比性的原则将其分为对照组与观察组,每组各161条次。对照组选择手工清洗,观察组选择自动软式内镜清洗消毒机清洗,分别选择ATP生物荧光测定法、管腔器械检测仪检查法对清洗效果进行评估,选择倾注培养法对内镜染菌情况进行检测以评估消毒质量,并观察记录工作人员躯体疲劳程度及内镜管道刷洗时间。结果观察组的ATP生物荧光测定法清洗质量合格率、管腔器械检测仪检查法清洗质量合格率均高于对照组,差异有统计学意义(P<0.05)。观察组的内镜腔道、阀门及表面消毒合格率高于对照组,差异有统计学意义(P<0.05)。观察组刷洗后工作人员的躯体疲劳程度评分低于对照组,差异有统计学意义(P<0.05);观察组的内镜管道刷洗时间长于对照组,差异有统计学意义(P<0.05)。结论在对内镜进行清洗消毒时,相比于手动清洗消毒的方式,自动软式内镜清洗消毒机对腔道的清洗消毒质量则更加理想,而且采用自动软式内镜清洗消毒机能对工作环境进行改善,让工作人员的健康风险降低,减少交叉感染概率,减轻工作人员的任务量及工作强度。 Objective To compare the cleaning and disinfection efficacy between automated flexible endoscope washer-disinfectors and manual cleaning methods,aiming to enhance endoscope cleaning quality and ensure patient and practitioner safety.Methods A total of 322 flexible endoscope cleaning cycles from January 2021 to June 2023 at a hospital's central sterile supply department were selected.Following the principle of comparability of baseline characteristics between groups,these were divided into a control group and an observation group,each comprising 161 cases.Manual cleaning was employed in the control group,the automated flexible endoscope washer-disinfector was used in the observation group.Cleaning efficacy was assessed using the ATP biofluorescence assay and lumen instrument inspection method.Endoscope contamination levels were evaluated via the pour plate method to gauge disinfection quality.Staff physical fatigue levels and endoscope channel scrubbing times were observed and recorded.Results The observation group demonstrated a statistically significantly higher rate of satisfactory cleaning quality than the control group,as measured by both the ATP biofluorescence assay and the lumen instrument inspection method.The observation group also achieved a statistically significantly higher rate of satisfactory disinfection of endoscope lumens,valves,and surfaces.Staff fatigue scores post-scrubbing were lower in the observation group than the control group,with statistically significant differences(P<0.05).Endoscope channel scrubbing duration was longer in the observation group than the control group,with statistically significant differences(P<0.05).Conclusion During the cleaning and disinfecting endoscopes,the automated flexible endoscope washer-disinfector achieves superior quality of lumen cleaning and disinfection compared to manual methods.Furthermore,the application could improve the working environment,reduces health risks for staff and the probability of cross-infection,and alleviates the workload and intensity of tasks for personnel.
