摘要
目的探讨以脑心健康管理师为主导的医护康一体化延续护理模式对脑卒中介入术后患者康复预后的影响。方法选取2024年1月至2025年2月韶关市第一人民医院神经内科收治的100例脑卒中介入术后患者为研究对象,采用区组随机化方法进行分组,通过SPSS 26.0生成随机数字序列,按照1∶1比例将患者分为观察组与对照组,各50例。对照组接受常规护理随访和康复指导,观察组采用以脑心健康管理师为主导的医护康一体化延续护理模式。比较两组患者干预前后神经功能、日常生活能力及生活质量的变化。结果干预3月后,观察组的美国国立卫生研究院卒中量表(NIHSS)评分低于对照组,改良Barthel指数评分显著高于对照组(P<0.001)。观察组生活质量量表(SF-36)得分显著高于对照组(P<0.001),观察组康复依从性明显优于对照组(P<0.05)。结论以脑心健康管理师为主导的医护康一体化延续护理模式能够改善脑卒中介入术后患者的神经功能恢复及日常生活能力,提高患者生活质量。
Objective To explore the impact of a brain-heart health manager-led integrated medical-nursing-rehabilitation continuing care model on rehabilitation outcomes in post-interventional stroke patients.Methods A total of 100 post-interventional stroke patients admitted to the Department of Neurology at the First People's Hospital of ShaoguanCIty from January 2024 to February 2025 were enrolled.Participants were randomly divided into an observation group and a control group(50 patients each)using block randomization via SPSS 26.0.The control group received routine nursing follow-up and rehabilitation guidance,while the observation group adopted the brain-heart health manager-led integrated continuing care model.Changes in neurological function,daily living ability,and quality of life were compared between the two groups before and after the intervention.Results After 3 months of intervention,the observation group showed significantly lower National Institutes of Health Stroke Scale(NIHSS)scores(P<0.001)and higher Modified Barthel Index(MBI)scores(P<0.001)compared to the control group.The observation group also demonstrated significantly improved 36-Item Short Form Health Survey(SF-36)scores(P<0.001)and better rehabilitation compliance(P<0.05).Conclusion The brain-heart health manager-led integrated continuing care model effectively improves neurological recovery,daily living ability,and quality of life in post-interventional stroke patients.
