摘要
目的探讨聚焦式心理支持联合健康教育对内镜黏膜下剥离术(ESD)患者的干预效果。方法选取郑州大学第二附属医院2024年1月至2024年12月收治的86例接受ESD治疗的消化道肿瘤患者纳入研究,按随机数字表法分为各43例的对照组(常规护理)与研究组(聚焦式心理支持联合健康教育+常规护理),比较两组知信行(KAP)水平、心理状态、自护能力、护理满意率及并发症发生率。结果术后1周,两组健康知识评分、健康信念评分、健康行为评分均较术前1天升高,且研究组高于对照组,差异有统计学意义(P<0.05)。术后1周,焦虑评分、抑郁评分均较术前1天降低,且研究组低于对照组,差异有统计学意义(P<0.05)。术后1周,两组自护能力评分均较术前1天升高,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组护理满意率高于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论在ESD围术期实施聚焦式心理支持联合健康教育,有助于提升患者的KAP水平,缓解其负面情绪,提高其自护能力与护理满意率,减少相关并发症发生。
Objective To investigate the intervention effect of focused psychological support combined with health education on patients undergoing endoscopic submucosal dissection(ESD).Methods A total of 86 patients with gastrointestinal tumors who received ESD in The Second Affiliated Hospital of Zhengzhou University from January 2024 to December 2024 were enrolled.According to the random number table method,they were randomly assigned to a control group(routine nursing,n=43)and a study group(focused psychological support combined with health education plus routine nursing,n=43).Levels of knowledge,attitude,and practice(KAP),psychological status,self-care ability,nursing satisfaction rate,and incidence of complications were compared between the two groups.Results At 1 week after surgery,scores for health knowledge,health belief,and health behavior in both groups were higher than those at 1 day before surgery,and these scores were higher in the study group than in the control group(P<0.05).At 1 week after surgery,anxiety scores and depression scores in both groups were lower than those at 1 day before surgery,and these scores were lower in the study group than in the control group(P<0.05).At 1 week after surgery,self-care ability scores in both groups were higher than those at 1 day before surgery,and these scores were higher in the study group than in the control group(P<0.05).The nursing satisfaction rate in the study group was higher than that in the control group(P<0.05).The incidence of postoperative complications in the study group was lower than that in the control group(P<0.05).Conclusion Perioperative implementation of focused psychological support combined with health education in patients undergoing ESD helps improve their KAP level,alleviate negative emotions,enhance self-care ability and nursing satisfaction,and reduce the incidence of related complications.
