摘要
目的探究转化生长因子-β1(TGF-β1)、干扰素-γ(IFN-γ)水平在支气管肺炎患儿中的表达及其对病情程度的评估价值。方法选取2023年6月至2024年6月于许昌市中心医院进行治疗的124例支气管肺炎患儿为研究对象,纳入研究组,并选取同期80例健康儿童作为对照组。根据《诸福棠实用儿科学(第7版)》中重症肺炎的通用判断标准将研究组患儿划分为轻症组(52例)和重症组(72例)。分别比较研究组患儿和对照组儿童、轻症组和重症组患儿的外周血TGF-β1、IFN-γ水平,Spearman分析研究组患儿外周血TGF-β1、IFN-γ水平与CPIS评分的相关性,受试者操作特征曲线(ROC曲线)分析外周血TGF-β1、IFN-γ水平评估患儿病情的效能。结果研究组患儿TGF-β1、IFN-γ水平均高于对照组,差异有统计学意义(P<0.05)。重症组患儿TGF-β1、IFN-γ水平和CPIS评分均高于轻症组,差异有统计学意义(P<0.05)。支气管肺炎患儿TGF-β1(r=0.458)和IFN-γ(r=0.370)水平均与CPIS评分呈正相关(P<0.05)。ROC曲线分析显示外周血TGF-β1、IFN-γ水平和二者联合均对支气管肺炎患儿病情有较高的预测价值,且二者联合的预测价值最高,曲线下面积(AUC)值分别为0.818、0.772、0.918。结论支气管肺炎患儿外周血TGF-β1、IFN-γ水平及CPIS评分水平升高,且重症患儿水平更高,联合检测TGF-β1、IFN-γ水平有助于患儿病情程度的早期评估。
Objective To investigate the expression levels of transforming growth factor-β1(TGF-β1)and interferon-γ(IFN-γ)in children with bronchopneumonia and to evaluate their value for assessing disease severity.Methods A total of 124 children with bronchopneumonia who received treatment in our hospital from June 2023 to June 2024 were enrolled as the study group,and 80 healthy children during the same period were selected as the control group.According to the general judgment criteria for severe pneumonia in“Zhu Futang’s Practical Pediatrics(7th Edition)”,the children in the study group were divided into the mild group(52 cases)and the severe group(72 cases).Peripheral blood TGF-β1 and IFN-γlevels were compared between the study and control groups and between the mild and severe groups.Spearman correlation analysis was used to assess the relationships between peripheral blood TGF-β1 and IFN-γlevels and CPIS scores in the study group.An ROC curve was plotted to compare the AUC of the single indicator versus the combined indicator.Results Peripheral blood TGF-β1 and IFN-γlevels in the study group were higher than those in the control group(P<0.05).Peripheral blood TGF-β1 and IFN-γlevels and CPIS scores in the severe group were higher than those in the mild group(P<0.05).Both TGF-β1(r=0.458)and IFN-γ(r=0.370)levels were positively correlated with CPIS scores in children with bronchopneumonia(P<0.05).ROC curve analysis showed that peripheral blood TGF-β1 and IFN-γlevels,as well as their combination,had high predictive value for disease severity in children with bronchopneumonia,with areas under the curve(AUCs)of 0.818,0.772,and 0.918,respectively.Conclusion The levels of TGF-β1 and IFN-γin the peripheral blood and the CPIS score of children with bronchopneumonia increase,and the levels are even higher in children with severe bronchopneumonia.The combined detection of TGF-β1 and IFN-γlevels is helpful for the early assessment of the severity of the disease in children.
