Ilizarov技术——胫骨横向骨搬移治疗糖尿病足临床疗效

摘要 目的本研究系统评估Ilizarov技术——胫骨横向骨搬移技术在糖尿病足患者临床治疗中的应用效果,为临床中治疗该疾病提供参考。方法选取10例确诊为糖尿病足的患者作为研究对象,均对其实施胫骨横向骨搬移手术治疗,对比术前、术后情况,观察其术前、术后患肢CTA检查。结果所有纳入研究的10例患者均成功完成胫骨横向骨搬移手术,术后未出现皮肤坏死现象,创面均实现顺利愈合,保肢率可达100%。统计显示,10例患者的创面愈合时间为12~24周,平均愈合时间为18周;术后随访期间未发生骨折、外固定架松动及其他相关并发症。对10例患者均完成术后6个月随访,影像学检查结果显示,胫骨搬移区域的骨折块复位良好,骨愈合进程符合预期。此外,患者术后动脉血管造影结果提示,下肢表浅动脉管径较术前明显增粗,侧支循环数量显著增加。结论胫骨横向骨搬移技术用于糖尿病足治疗,可有效改善患肢组织微循环,缓解患者疼痛症状,显著提高临床保肢率,具备较高的临床推广价值。需注意的是,在临床应用该手术方案时,需严格参照手术适应症对患者进行筛选,以确保治疗安全性与有效性。 Objective This study aims to systematically evaluate the application effect of Ilizarov-tibial transverse bone transport technique in the clinical treatment of patients with diabetic foot,and to provide a reference for the clinical treatment of this disease.Methods Ten patients diagnosed with diabetic foot were selected as the research subjects.All of them were treated with this surgical procedure.Compare the preoperative and postoperative conditions,and the CTA examinations of the affected limbs before and after the operation were observed.Results All 10 patients included in the study successfully completed the transverse tibial bone transfer surgery.No skin necrosis occurred after the operation,and the wounds healed smoothly.The limb salvage rate could reach 100%.Statistics show that the wound healing time of 10 patients ranged from 12 to 24 weeks,with an average healing time of 18 weeks.No fractures,loosening of the external fixator or other related complications occurred during the postoperative follow-up period.All 10 patients were followed up for 6 months after the operation.The imaging examination results showed that the fracture fragments in the tibial transfer area were well reduced,and the bone healing process was in line with expectations.In addition,the postoperative arterial angiography results of the patient indicated that the diameter of the superficial arteries in the lower extremities was significantly thicker than that before the operation,and the number of collateral circulations increased remarkably.Conclusion This technology can effectively improve the microcirculation of affected limb tissues,relieve the pain symptoms of patients,significantly increase the clinical limb preservation rate,and has high clinical promotion value.It should be noted that when applying this surgical plan in clinical practice,patients must be strictly screened in accordance with the surgical indications to ensure the safety and effectiveness of the treatment.
