摘要
目的 探究儿童腹部CT检查中器官剂量及其与体型之间的相关性。方法 回顾性收集3415例天津市儿童医院根据体重分组行腹部常规CT检查的患儿资料,应用Radimetrics软件计算患儿不同器官当量剂量(H)再进一步根据腹部扫描范围将其分为扫描范围内器官(IO)、周围器官(PO)、分布式器官(DO)、扫描范围外器官(OO)。非参数方差分析对组间及组内器官剂量组(ODG)进行比较,采用KruskalWallis检验进行事后检验。以代表体型的水当量直径(Dw)作为自变量,IO剂量作为因变量,CT剂量指数(CTDI)、年龄、性别作为协变量构建相应线性回归模型。结果 多数组间比较中差异存在统计学意义,组内IO、PO、DO、OO剂量差异存在统计学意义,且为IO>DO>PO>OO。Dw与IO剂量间存在负相关(β=-0.242,R^(2)=0.768,P<0.001)。结论 制定扫描协议时考虑体型因素可以降低儿童尤其是小体型儿童的辐射风险暴露。
Objective To explore the organ dose in abdominal computed tomography(CT)examination of children and its correlation with body size.Methods The data of 3415 children who underwent abdominal routine CT examination in Tianjin Children's Hospital according to body weight were retrospectively collected.Radimetrics software are used to calculate different organs equivalent dose(H)and further it can be divided into inside organs(IO),peripheral organs(PO),distributed organs(DO),the outside organs(OO)according to the scope of the abdominal scan range.Organ dose groups(ODG)for among groups and intra-group were compared by non-parametric analysis of variance and post-hoc tests were performed using the Kruskal-Wallis test.A linear regression model was constructed with water equivalent diameter(Dw)representing body size as the independent variable,IO dose as the dependent variable,CT dose index(CTDI),age and gender as covariates.Results Comparison between the majority groups differences are statistically significant.There were significant differences in the doses of IO,PO,DO and OO among groups,which were IO>DO>PO>OO.A negative correlation was between Dw and IO dose(β=-0.242,R^(2)=0.768,P<0.001).Conclusion Considering the size factor in the scanning protocol can reduce the radiation risk exposure of children,especially small size children.
