摘要
目的 回顾分析“开始正确”理念在发热门诊临床管理和医学教育中的应用价值及实践成效。方法 基于“开始正确”理念建立发热门诊规范化诊疗流程和分级管理体系,抽取2022年1月—2022年6月广东省中医院大学城医院发热门诊接诊的150例具有传染病流行病学史的就诊者,采用事后分析法评价预检分诊风险人群识别正确率;制定发热门诊危重病种诊治方案,实施案例分析式带教,将2021年1月—2022年6月发热门诊接诊的40名梗阻性肾病患者分为对照组和实践组,每组20例,通过比较2组患者接诊后完成CT检查时间、专科会诊时间和转送病房时间,评估带教成效。结果 临床管理实践方面,预检分诊风险识别准确率达100%;临床带教实践方面,与对照组比较,实践组患者完成CT检查、专科会诊和转送至病房的时间均缩短(P<0.05)。结论 “开始正确”理念能指导发热门诊诊疗流程优化,提高急危重症识别效率,改善医疗服务质量,在临床管理和医学教育中均具有重要的应用价值。
Objective To retrospectively analyze the application value and practical effectiveness of the“Start Correctly”concept in clinical management and medical education in fever clinics.Methods Based on the“Start Correctly”concept,standardized diagnosis and treatment procedures and a hierarchical management system were established for fever clinics.A total of 150 patients with epide⁃miological history who visited the fever clinic of Guangdong Provincial Hospital of Traditional Chinese Medicine from January to June 2022 were selected,and the accuracy rate of risk stratification in pre-examination and triage was evaluated using retrospective analy⁃sis.Critical disease diagnosis and treatment protocols for fever clinics were developed,and case-based teaching was implemented.Forty patients with obstructive nephropathy treated in the fever clinic from January 2021 to June 2022 were divided into a control group and a practice group,with 20 cases in each group.The teaching effectiveness was evaluated by comparing the time to complete CT examination,specialist consultation time,and ward transfer time after admission between the two groups.Results In terms of clini⁃cal management practice,the accuracy rate of risk identification in triage reached 100%.In terms of clinical teaching practice,com⁃pared with the control group,the time for patients in the experimental group to complete CT examination,the time for specialist consul⁃tation,and the time for transfer to the ward were all shortened(P<0.05).Conclusion The concept of“Start Correctly”can optimize the diagnosis and treatment procedures in fever clinics,improve the efficiency of identifying critical and severe cases,enhance the quality of medical services,and has significant application value in clinical management and medical education.
