摘要
目的探讨血清CXC趋化因子配体10(CXCL10)、CXC趋化因子配体12(CXCL12)和热休克蛋白70(HSP70)水平在重症急性胰腺炎(SAP)患者预后预测中的应用价值。方法选取90例SAP患者,将其分为预后良好组(n=59)和预后不良组(n=31),检测患者入院时血清CXCL10、CXCL12和HSP70的水平。采用多因素logistic回归分析影响SAP患者预后的独立危险因素,使用受试者工作特征(ROC)曲线评估这些指标对SAP患者预后的判断效能。结果90例SAP患者中,有31例预后不良,59例预后良好。预后不良组患者急性生理与慢性健康评分系统Ⅱ(APACHEⅡ)评分、白细胞计数(WBC)、降钙素原(PCT)、C反应蛋白(CRP)、血清CXCL10、CXCL12和HSP70水平高于预后良好组(P<0.05)。多因素logistic回归结果表明,APACHEⅡ评分、CXCL10、CXCL12和HSP70均为影响SAP患者预后的独立危险因素(P<0.05)。ROC曲线结果显示,CXCL10、CXCL12和HSP70预测SAP患者预后不良的曲线下面积(AUC)分别为0.824、0.701和0.776,而当选用CXCL10、CXCL12和HSP70进行联合预测时,AUC值则达到0.904。结论血清CXCL10、CXCL12和HSP70水平在重症急性胰腺炎患者的预后预测中具有重要价值,联合检测可提高预后判断的准确性。
Objective To investigate the predictive value of serum levels of CXC chemokine ligand 10(CXCL10),CXC chemokine ligand 12(CXCL12),and heat shock protein 70(HSP70)in the prognosis of patients with severe acute pancreatitis(SAP).Methods90 patients with SAP were selected,and the serum levels of CXCL10,CXCL12,and HSP70 were measured upon admission.Multivariate logistic regression analysis was performed to identify independent risk factors affecting the prognosis of SAP patients.Receiver operating characteristic(ROC)curves were used to evaluate the predictive efficacy of these indicators for SAP prognosis.Results Among the 90SAP patients+3l patients had a poor prognosis and 59 patients had a good prognosis.The poor prognosis group had higher Acute Physiology and Chronic Health Evaluation II(APACHEⅡ)scores,white blood cell count(WBC),procalcitonin(PCT),C-reactive protein(CRP),and serum levels of CXCL10,CXCL12,and HSP70 compared to the good prognosis group(P<0.05).Multivariate logistic regression showed that APACHEⅡI score,CXCL10,CXCL12,and HSP70 were independent risk factors for the prognosis of SAPpatients(P<0.05).ROC curve analysis showed that the areas under the curve(AUC)for CXCL10,CXCL12,and HSP70 in predicting poor prognosis in SAP patients were 0.824,0.701,and 0.776,respectively.When CXCL10,CXCL12,and HSP70 were used in combination,the AUC reached 0.904.Conclusion Serum levels of CXCL10,CXCL12,and HSP70 have significant prognostic value in SAPpatients,and combined testing can improve prognostic accuracy.
