血清miR-21、miR-155与Gal-3对老年心力衰竭患者诊断及预后的相关性研究

The correlation between serum miR-21,miR-155,and Gal-3 in the diagnosis and prognosis of elderly patients with heart failure

摘要 目的探讨血清微小RNA-21(miR-21)、微小RNA-155(miR-155)与半乳糖凝集素-3(Gal-3)在老年心力衰竭(HF)患者中的诊断及预后价值。方法选取老年HF患者142例作为HF组,选取148例健康人作为健康组。对比2组研究对象左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、氨基末端B型脑利钠肽前体(NT-proBNP)、高敏C反应蛋白(hs-CRP)、miR-21、miR-155、Gal-3水平差异;将HF组患者根据心功能分级分为Ⅱ级组(44例)、Ⅲ级组(77例)、Ⅳ级组(21例),对比3组患者相关资料;采用Pearson相关系数分析HF组患者血清miR-21、miR-155、Gal-3与LVEF、LVEDD、NT-proBNP、hs-CRP的相关性;对HF组患者随访6个月,根据随访结果将其分为预后良好(94例,未发生心脏不良事件)与预后不良(48例,发生心脏不良事件),采用多因素logistic回归分析影响HF患者预后的危险因素。结果HF组患者血清miR-21、miR-155、Gal-3水平高于健康组(P<0.05),且HF组心功能Ⅱ级、Ⅲ级、Ⅳ级组患者中,血清miR-21、miR-155、Gal-3水平随分级升高而升高(P<0.05);血清miR-21、miR-155、Gal-3与LVEF呈负相关,与LVEDD、NT-proBNP、hs-CRP呈正相关(P<0.05);血清miR-21、miR-155、Gal-3是影响HF患者预后的独立危险因素(P<0.05)。结论血清miR-21、miR-155和Gal-3在HF患者中显著升高,可作为HF诊断及预后评估的生物标志物。 Objective To explore the value of serum microRNA-21(miR-21),microRNA-155(miR-155)and galectin-3(Gal-3)in the diagnosis and prognosis in elderly patients with heart failure(HF).Methods142 elderly patients with HF were selected as HF group,and 148 healthy people were included in healthy group.The differences in left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),N-terminal pro-brain natriuretic peptide(NT-proBNP),high-sensitiv-ity C-reactive protein(hs-CRP),miR-21,miR-155 and Gal-3 were compared between the two groups.The patients in HF group were divided into grade II group(n=44),grade Igroup(n=77)and grade IV group(n=21)according to cardiac function grading.The related data of patients were compared among the three groups.Pearson correlation coefficient was used to analyze the correlation between serum miR-21,miR-155,Gal-3 and LVEF,LVEDD,NT-proBNP,hs-CRPin HF group.Patients in HF group were followed up for 6 months,and were classified into good prognosis group(n=94,without adverse cardiac events)and poor prognosis group(n=48,with adverse cardiac events)according to the follow-up results,and the risk factors affecting the prognosis in HF patients were analyzed by multivariate logistics regression analysis.Results Serum miR-21,miR-155 and Gal-3 levels in HF group were higher than those in healthy group(P<0.05).Among gradeⅡgroup.grade lgroup and grade IV group in HF group,serum miR-21,miR-155 and Gal-3 were increased with the increase of grade(P<0.05).Serum miR-21,miR-155and Gal-3 were negatively correlated with LVEF,and were positively correlated with LVEDD,NT-proBNPand hs-CRP(P<0.05).Serum miR-21,miR-155 and Gal-3 were independent risk factors affecting the prognosis in HF patients(P<0.05).Conclusion Serum miR-21,miR-155and Gal-3 are significantly increased in HF patients,and they can be used as biomarkers for the diagnosis and prognosis evaluation of HF.
