摘要
术后胃肠功能紊乱(PGID)作为现代手术诊疗方式的常见并发症,主要表现为腹胀、恶心呕吐(PONV)、疼痛、排气延长等,严重者可导致患者出现感染、肠梗阻、肠粘连等。中医外治法对PGID的起效机制具有多途径、多靶点的特点,可通过调节食欲素A(OXA)信号通路、血清炎症介质水平、5-羟色胺(5-HT)信号系统、胃肠道Cajal间质细胞(ICCs)相关因子、血清miR-222、miR-19a、胃肠道相关激素水平、脑-肠肽等治疗PGID;治疗PGID的常用中医外治法包括针刺(传统针刺、电针、火针、脐针、揿针、腕踝针)、艾灸(温针灸、隔姜灸、脐灸等)、穴位注射、穴位贴敷、耳穴、推拿等。
Postoperative gastrointestinal disorders(PGID),as a common complication of modern surgical diagnosis and treatment methods,mainly manifest as abdominal distention,nausea and vomiting(PONV),pain,prolonged flatulence,and other symptoms.In severe cases,it can lead to complications,such as infection,intestinal obstruction,and intestinal adhesion,in patients.The mechanism of action of external treatment methods in traditional Chinese medicine on PGID exhibits multi-pathway and multi-target characteristics.PGID can be effectively treated by regulating the orexin A(OXA)signaling pathway,serum inflammatory mediator levels,the 5-hydroxytryptamine(5-HT)signaling system,gastrointestinal Cajal interstitial cell(ICC)-related factors,serum miR-222 and miR-19a levels,gastrointestinal-related hormones,and brain-gut peptides.Common external treatment methods in traditional Chinese medicine for PGID encompass acupuncture(such as traditional acupuncture,electroacupuncture,fire acupuncture,umbilical acupuncture,press acupuncture,and wrist-ankle acupuncture),moxibustion(such as warming needle moxibustion,ginger moxibustion,and umbilical moxibustion),acupoint injection,acupoint application,auricular acupuncture,massage,and other techniques.
