无创呼吸机联合氧驱动雾化吸入对AECOPD伴呼吸衰竭患者血气指标及预后的影响

摘要 目的:探究无创呼吸机联合氧驱动雾化吸入对慢性阻塞性肺疾病急性加重期(AECOPD)伴呼吸衰竭患者血气指标及预后的影响。方法:选取2022年1月~2023年12月本院65名AECOPD伴呼吸衰竭患者,采用随机数字表法分为对照组32例与联合组33例。对照组采用无创呼吸机单独治疗,联合组在对照组基础上进行氧驱动雾化吸入。比较两组患者临床疗效、血气指标[血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、炎症指标[白细胞介素-6(IL-6)、降钙素原(PCT)、C反应蛋白(CRP)]、预后情况差异。结果:治疗后,联合组临床疗效总有效率高于对照组(P<0.05);治疗后,两组患者SaO_(2)、PaO_(2)均升高,联合组高于对照组,两组PaCO_(2)、IL-6、PCT、CRP均降低,且联合组低于对照组(P<0.05);随访1年,联合组预后良好人数高于对照组,差异具有统计学意义(P<0.05)。结论:在伴有呼吸衰竭的AECOPD患者中,无创呼吸机与氧驱动雾化吸入结合使用,可以减轻患者的炎症状态,恢复血气指标平衡,与单独无创呼吸机相比提高临床疗效。 Objective:To investigate the effect of non-invasive ventilator combined with oxygen driven nebulization inhalation on blood gas indicators and prognosis in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)accompanied by respiratory failure.Methods:A total of 65 AECOPD patients with respiratory failure in our hospital from January 2022 to December 2023 were selected and divided into control group(32 cases)and combined group(33 cases)by random number table method.The control group was treated with non-invasive ventilator alone,and the combined group was treated with oxygen-driven atomization inhalation on the basis of the control group.Compare the clinical efficacy,blood gas indicators(SaO_(2),PaO_(2),PaCO_(2)),inflammatory indicators(IL-6,PCT,CRP),and prognosis differences between two groups of patients.Results:After treatment,the total effective rate of clinical efficacy in the combination group was higher than that in the control group(P<0.05);After treatment,both groups of patients showed an increase in SaO_(2) and PaO_(2),with the combined group being higher than the control group.PaCO_(2),IL-6,PCT,and CRP in both groups decreased,with the combined group being lower than the control group(P<0.05);After a one-year follow-up,the number of patients with good prognosis in the combination group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:In AECOPD patients with respiratory failure,the combination of non-invasive ventilator and oxygen driven nebulization inhalation can alleviate the patient’s inflammatory state,restore blood gas balance,and improve clinical efficacy compared to non-invasive ventilator alone.
