摘要
随着社会发展,人们生活观念的转变,宠物犬数量不断增加,犬咬伤发生率也逐年增高。犬咬伤作为急诊常见致伤因素之一,除外科损伤外,还伴有破伤风、狂犬病、感染等严重的并发症,伤后产生的瘢痕、预后不佳所致的容貌破坏都对患者的生活和心理造成极大的负担,犬咬伤已然成为一项严重的公共卫生问题。犬咬伤创口的诊疗关键在于对创面的早期干预,最重要的就在于创口的彻底消毒清创、创口的闭合以及感染的预防。目前,创口闭合方式主要可分为Ⅰ期闭合与延期闭合两种方式,在其诊疗过程中,创口闭合方式的选择仍然存在争议,伤口感染率、伤口瘢痕形成情况、伤口愈合后美观程度等都成为影响创口闭合方式选择的因素。现对犬咬伤创口不同闭合方式的优劣作一综述,为临床治疗方式的选择提供参考和依据。
With the development of the nation and the change in people's lifestyle,the number of pet dogs is increasing,leading to a rise in the incidence of dog bites year by year.Dog bites,as one of the common causes of injury in emergency departments,not only result in surgical trauma but also are accompanied by serious complications such as tetanus,rabies,and infections.The scars and disfigurement caused by poor prognosis after the injury can impose a significant psychological and social burden on the patients.Therefore,dog bites have become a serious public health issue.The key to the diagnosis and treatment of dog bite wounds lies in early intervention,with thorough debridement and disinfection of the wound,wound closure,and prevention of infection being most crucial.Currently,wound closure methods can be mainly divided into primary closure and delayed closure.However,there is still controversy over the choice of wound closure method in clinical practice.Factors such as wound infection rate,scar formation,and aesthetic outcome after wound healing all influence the selection of the closure method.This review aims to explore the advantages and disadvantages of different closure methods for dog bite wounds,providing a reference and basis for the selection of clinical treatment methods.
