摘要
顽固性呃逆是脑卒中后较常见并发症,不仅不利于脑卒中恢复期的康复,甚至可严重影响患者的身心健康及生活。形气神一体观源于《淮南子》,属中医理论从物质形态、能量功能、精神思维三个层面认识人体生理和病理的生命观。中风后顽固性呃逆发病兼具“形气神同病”的特征,其核心病机涉及筋脉瘛疭、膈肌痉挛之形痉,肝胃失和、胃气上逆之气逆,脑髓损伤、神机失和之神郁,治疗则以舒肌安膈、复形之位,疏肝和胃、降气之逆,益精填髓、解郁调神为要,结合治呃要穴攒竹,最终达到恢复形气神三位一体稳态的目的。
Intractable hiccups are a relatively common complication after stroke.They not only hinder rehabilitation during the recovery period but can also severely impact the patient's body and mental health as well as their quality of life.The holistic view of body-qi-spirit,originating from the ancient text Huai Nan Zi(The Writings of the Prince of Huainan),is a life view in Chinese medicine that understands human physiology and pathology through three interconnected levels:material form(body),energy function(qi),and mental-spiritual activity(spirit).Post-stroke intractable hiccups present with characteristics of"simultaneous disorder of the body,qi,and spirit."Its core pathogenesis is believed to involve:body spasm,manifesting as clonic convulsion of sinews and vessels and diaphragmatic spasm;reversed flow of qi:involving disharmony between the liver and stomach,leading to reversed flow of stomach qi;and spirit constraint:resulting from brain marrow impairment and dysregulation of the spirit activity.The treatment principles should be focusing on relaxing muscles and calming the diaphragm to restore the body's normal state;soothing the liver and harmonizing the stomach to direct qi downward;and replenishing essence and enriching the marrow to relieve constraint and regulate the spirit.This approach,combined with needling the primary acupoints for hiccups such as Cuanzhu(BL2),ultimately aims to restore the steady state of holistic view of body-qi-spirit.
