摘要
目的观察利咽败毒汤联合氧喷治疗鼻咽癌患者放射性口咽炎的疗效。方法选取2021年3月至2024年5月河北省沧州中西医结合医院的100例鼻咽癌患者,随机分为治疗组50例和对照组50例。治疗组采用利咽败毒汤联合氧喷治疗,对照组采用康复新液口服治疗。比较两组患者发生口腔黏膜损伤的时间、出现该反应时所对应的放射累积剂量、口腔黏膜反应分度、唾液腺损伤程度及治疗前后卡氏功能状态评分(karnofsky performance status,KPS)变化情况。结果治疗组与对照组比较,治疗组出现口腔黏膜反应的时间较对照组晚(P<0.05);治疗组出现口腔黏膜反应的累积放射剂量较对照组多(P<0.05)。当放疗剂量为50Gy及放疗结束时,两组出现口腔黏膜反应分度比较差异有统计学意义(P<0.05),两组患者唾液腺损伤程度比较差异有统计学意义(P<0.05)。两组治疗后KPS评分大部分均较治疗前改善,但利咽败毒汤组生存质量高于对照组,且差异有统计学意义(P<0.05)。结论利咽败毒汤联合氧喷能可延迟鼻咽癌患者急性放射性口腔黏膜反应的初次出现时间,有效减轻鼻咽癌患者放射性口腔黏膜损伤及唾液腺损伤程度,提高生存质量。
Objective To observe the efficacy of Liyan Baidu Decoction combined with oxygen spray in nasopharyngeal carcinoma(NPC)patients with radiation-induced oropharyngitis.Methods A total of 100 NPC patients with radiation-induced oropharyngitis treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei Province from March 2021 to May 2024 were selected and randomly divided into a treatment group of 50 cases and a control group of 50 cases.The treatment group was treated with Liyan Baidu Decoction combined with oxygen spray,while the control group was treated with oral administration of Kangfuxin Liquid.The time of oral mucosal injury occurrence,the corresponding cumulative radiation dose at the time of reaction onset,grading of oral mucosal reaction,degree of salivary gland injury,and changes in karnofsky performance status(KPS)score before and after treatment were compared between the two groups.Results Compared with the control group,the treatment group had a later onset time of oral mucosal reaction(P<0.05)and a higher cumulative radiation dose at the time of reaction occurrence(P<0.05).When the radiation dose reached 50Gy and at the end of radiotherapy,there were statistically significant differences in the grading of oral mucosal reaction(P<0.05)and the degree of salivary gland injury(P<0.05)between the two groups.The KPS scores of both groups were mostly improved after treatment,but the quality of life in the Liyan Baidu Decoction group was higher than that in the control group,with a statistically significant difference(P<0.05).Conclusion Liyan Baidu Decoction combined with oxygen spray can delay the initial occurrence time of acute radiation-induced oral mucosal reaction in patients with nasopharyngeal carcinoma,effectively alleviate the degrees of radiation-induced oral mucosal injury and salivary gland injury,and improve the quality of life.
