摘要
结直肠癌作为我国发病率第二、死亡率第四的恶性肿瘤,严重威胁国民健康。现代医学的手术、放疗、化疗及靶向治疗等手段虽疗效确切,但仍存在癌前病变进程难阻断、放、化疗毒副作用显著、术后复发转移率高、晚期姑息治疗疗效有限等难点问题。中医药在抑制结直肠腺瘤复发与癌变、协同放化疗减轻毒副反应、预防肿瘤复发与转移、改善晚期患者生活质量并延长其生存期等方面作用突出。因此,构建中西医结合防治结直肠癌模式,实现优势互补,是临床发展的必然趋势。本文系统梳理结直肠癌发生发展四大关键阶段,包括癌前病变期、协同减毒期、术后防复期及带瘤生存期,以及中西医视角下的发病机制、治则治法、机制研究、临床研究与指南共识,旨在阐明中医药在各阶段结直肠癌治疗中的协同与补充价值,以期为中西医结合分期防治结直肠癌提供理论支撑与实践指导。
Colorectal cancer,ranking as the second most common malignant tumor in incidence and the fourth in mortality in China,poses a serious threat to public health.Although modern medical treatments such as surgery,radiotherapy,chemotherapy,and targeted therapy have demonstrated efficacy,they still face challenges including difficulties in blocking the progression of precancerous lesions,significant toxic side effects from radiotherapy and chemotherapy,high rates of postoperative recurrence and metastasis,and limited effectiveness of palliative care for advanced stages.Traditional Chinese medicine has shown prominent effects in inhibiting adenoma recurrence and carcinogenesis,reducing toxic side effects in combination with radiotherapy and chemotherapy,preventing tumor recurrence and metastasis,and improving the quality of life and prolonging survival for advanced patients.Therefore,establishing an integrated traditional Chinese and Western medicine model for the prevention and treatment of colorectal cancer,leveraging the strengths of both approaches,is an inevitable trend in clinical development.This article systematically reviews the pathogenesis,treatment principles and methods,mechanism research,clinical studies,and guidelines/consensus from the perspectives of both traditional Chinese and Western medicine across four key stages of colorectal cancer development(precancerous lesion stage,toxicity reduction during combined therapy,postoperative recurrence prevention,and living with tumor).The aim is to elucidate the synergistic and complementary value of traditional Chinese medicine in the treatment of colorectal cancer at each stage,providing theoretical support and practical guidance for the staged prevention and treatment of colorectal cancer with integrated traditional Chinese and Western medicine.
