摘要
目的分析血清基质金属蛋白酶(MMP)-3和核因子kB受体激活因子(RANK)水平与口腔种植修复效果的相关性。方法回顾性选取100例牙列缺失需进行口腔种植修复者作为实验组,依据种植6个月后修复效果分为良好组(72例)及不良组(28例),同期100例健康体检者作为对照组。收集受检者一般资料,采用酶联免疫吸附法检测血清MMP-3和RANK水平,比较不同修复效果患者的一般资料及血清MMP-3和RANK水平,多因素Logistic回归分析牙列缺失患者口腔种植修复效果的影响因素,采用受试者工作特征(ROC)曲线分析血清MMP-3、RANK水平对口腔种植修复效果的预测价值。结果实验组血清MMP-3和RANK水平高于对照组(P<0.05),不良组血清MMP-3和RANK水平高于良好组(P<0.05)。血清MMP-3和RANK水平是口腔种植效果的影响因素(P<0.05)。血清MMP-3和RANK水平单个指标单独预测口腔种植修复效果的AUC分别为0.914及0.911,联合预测的AUC为0.985,高于单独预测的AUC(P<0.05)。结论牙列缺失患者血清MMP-3和RANK水平均会升高,联合监测MMP-3和RANK水平可更好地预测患者的口腔种植修复效果。
Objective To analyze the correlation between serum matrix metalloproteinase-3(MMP-3)and receptor activator of nuclear factor kappa-B(RANK)levels and the repair effect of oral implantation in patients with missing dentition.Methods One hundred patients with missing dentition requiring oral implant restoration were retrospectively selected as the experimental group.According to the implant repair outcomes at 6-month follow-up,the patients were divided into a good-outcome group(n=72)and a poor-outcome group(n=28).Another 100 healthy individuals who underwent physical examination during the same period were included as the control group.General clinical data were collected,and serum MMP-3 and RANK levels were measured by enzyme-linked immunosorbent assay(ELISA).Differences in general data and serum MMP-3 and RANK levels among the groups were compared.Multivariate logistic regression analysis was performed to identify factors influencing the repair effect of oral implantation in patients with missing dentition.Receiver operating characteristic(ROC)curve analysis was used to evaluate the predictive value of serum MMP-3 and RANK levels for oral implantation repair effect.Results Serum MMP-3 and RANK levels in the experimental group were higher than those in the control group(P<0.05),and the poor-outcome group showed higher serum MMP-3 and RANK levels than the good-outcome group(P<0.05).Serum MMP-3 and RANK levels were influencing factors for the repair effect of oral implantation(P<0.05).The AUCs of serum MMP-3 and RANK for predicting the repair effect of oral implantation were 0.914 and 0.911,respectively,and the combined AUC was 0.985,which was higher than the AUCs of the individual indicators(P<0.05).Conclusion Serum MMP-3 and RANK levels increase in patients with missing dentition,and combined monitoring of MMP-3 and RANK levels can better predict the repair effect of oral implantation.
