FBXO43表达与头颈部鳞癌患者的预后及肿瘤免疫浸润的关系

Prognostic Effect of FBXO43 Expression in Head and Neck Squamous Cell Carcinoma and Its Relationship with Tumor Immune Invasion

摘要 目的探讨FBXO43表达与头颈部鳞癌患者预后及肿瘤免疫浸润的关系。方法选取头颈部鳞癌患者108例,均收集头颈部鳞癌组织、正常对照黏膜组织,免疫组化染色检测组织中FBXO43表达及CD3、CD4、CD8、Foxp3、CD80、CD86表达,FBXO43阳性表达与头颈部鳞癌者临床病理指标的关系,FBXO43表达与患者预后的关系,COX比例风险回归模型分析影响患者预后因素,头颈部鳞癌组织膜CD276表达与FBXO43表达、TILS的关系。结果头颈部鳞癌组织中FBXO43阳性表达率(53.70%)高于正常对照黏膜组织中其阳性表达率(32.41%)(P<0.05);不同分化程度、T分期、N分期、临床TNM分期患者FBXO43阳性表达比较差异显著(P<0.05);FBXO43阳性表达患者总生存率低于FBXO43阴性表达患者(Log-rankχ^(2)=4.884,P=0.027);COX比例风险回归模型分析,结果显示临床分期Ⅲ~Ⅳ期[OR=1.321,95%CI:1.151~2.685]、FBXO43阳性表达[OR=13.144,95%CI:4.083~22.982]均为影响患者预后危险因素(P<0.01);膜CD276^(+)表达与FBXO43表达上升、Foxp3+表达上升有关(P<0.05)。结论FBXO43的高表达与头颈部鳞癌患者的不良预后相关,并且其表达水平与肿瘤免疫浸润状态密切相关,尤其是与免疫抑制性T细胞Foxp3的表达增加相关,提示FBXO43可能作为潜在的生物标志物。 Objective To investigate the prognostic role of FBXO43 expression in head and neck squamous cell carcinoma and its relationship with tumor immune invasion.Methods A total of 108 patients with head and neck squamous cell carcinoma were selected.The tissues of head and neck squamous cell carcinoma and normal control mucosal tissues were collected.The expressions of FBXO43 and CD3,CD4,CD8,Foxp3,CD80 and CD86 in the tissues were detected by immunohistochemical staining.The relationship between positive expression of FBXO43 and clinicopathological indicators in patients with head and neck squamous cell carcinoma,the relationship between FBXO43 expression and patient prognosis,the factors affecting patient prognosis,the relationship between CD276 expression in tissue membrane of head and neck squamous cell carcinoma and TILS were analyzed by COX proportional risk regression model.Results The positive expression rate of FBXO43 in head and neck squamous cell carcinoma tissues was 53.70%,which was higher than that in normal control mucosal tissues(32.41%)(P<0.05).The positive expression of FBXO43 in patients with different differentiation degree,T stage,N stage and clinical TNM stage was significantly different(P<0.05).The overall survival rate of patients with positive expression of FBXO43 was lower than that of patients with negative expression of FBXO43(Log-rankχ^(2)=4.884,P=0.027).COX proportional hazard regression model analysis showed that clinical stage Ⅲ~Ⅳ[OR=1.321,95%CI:1.151~2.685],positive expression of FBXO43[OR=13.144,95%CI:4.083~22.982]were risk factors for prognosis(P<0.01).The expression of CD276^(+)was correlated with the increased expression of FBXO43 and Foxp^(3+)(P<0.05).Conclusion High expression of FBXO43 is associated with poor prognosis in patients with head and neck squamous cell carcinoma,and its expression level is closely related to tumor immunoinvasive state,especially the increased expression of immunosuppressive T cell Foxp3,suggesting that FBXO43 may serve as a potential biomarker.
