摘要
目的:探讨高频超声(HFUS)诊断婴幼儿髋关节发育不良(DDH)的有效性。方法:选取北京航天总医院2024年1—12月接诊的疑似DDH患儿100例,均行X线、HFUS检查,以临床综合检查结果为“金标准”,比较X线与HFUS诊断DDH的效能及二者对DDH类型的检出率。结果:临床综合检查示,100例疑似患儿中确诊DDH 80例。X线检查与“金标准”结果的一致性中等(Kappa=0.673,P<0.001),HFUS检查与“金标准”的一致性较好(Kappa=0.908,P<0.001)。HFUS诊断DDH的灵敏度、准确度高于X线(P=0.016)。HFUS对GrafⅡb、Ⅱc型检出率高于X线(P<0.05)。HFUS对髋臼发育缺陷的检出率高于X线(P<0.001)。结论:HFUS检查与“金标准”的一致性较好,HFUS检查婴幼儿DDH的准确度、灵敏度高于X线,并能准确鉴别DDH类型。
Objective:To explore the effectiveness of high-frequency ultrasound(HFUS)in diagnosing hip dysplasia(DDH)in infants and young children.Methods:One hundred children with suspected DDH who were admitted to Beijing Aerospace General Hospital from January to December 2024 were selected and all underwent X-ray and HFUS examinations.Taking the results of comprehensive clinical examinations as the"gold standard",the efficacy of X-ray and HFUS in diagnosing DDH and their detection rates for DDH types were compared.Results:Comprehensive clinical examination showed that among 100 suspected children,80 cases were diagnosed with DDH.The consistency between X-ray examination and the"gold standard"results was moderate(Kappa=0.673,P<0.001),while the consistency between HFUS examination and the"gold standard"was relatively good(Kappa=0.908,P<0.001).The sensitivity and accuracy of HFUS in diagnosing DDH were higher than those of X-ray(P=0.016).The detection rate of Graf typesⅡb andⅡc by HFUS was higher than that by X-ray(P<0.05).The detection rate of acetabular developmental defects by HFUS was higher than that by X-ray(P<0.001).Conclusion:The consistency between HFUS examination and the"gold standard"is relatively good.The accuracy and sensitivity of HFUS examination for DDH in infants and young children are higher than those of X-rays,and it can accurately identify the type of DDH.
