摘要
目的:分析大柴胡汤加减联合注射用西维来司他钠治疗脓毒症的效果。方法:选取东莞市中西医结合医院2023年6月—2025年6月收治的脓毒症患者72例,以掷硬币法分为常规组和试验组,各36例。常规组在常规西医治疗基础上使用注射用西维来司他钠治疗,试验组在常规组基础上联合大柴胡汤加减治疗。对比两组应用效果。结果:治疗后,试验组神昏乏力、胸腹刺痛、四肢厥冷、高热评分低于常规组(P<0.001);治疗后,试验组白细胞介素-6、降钙素原水平、C反应蛋白、白细胞计数水平低于常规组(P<0.05);治疗后,试验组免疫球蛋白A、免疫球蛋白G、免疫球蛋白M水平低于常规组(P<0.05);试验组机械通气时间、ICU住院时间短于常规组(P<0.001),住院费用少于常规组(P<0.001);两组28 d死亡率对比,无统计学差异(P>0.05)。结论:大柴胡汤加减联合注射用西维来司他钠治疗脓毒症的效果显著,可缓解症状,减轻炎症,改善免疫功能,缩短治疗周期,减少治疗费用。
Objective:To analyze the effect of modified Dachaihu decoction combined with sivelestat sodium for injection in treatment of sepsis.Methods:A total of 72 sepsis patients admitted to Dongguan Hospital of Integrated Traditional Chinese and Western Medicine from June 2023 to June 2025 were selected.They were divided into the conventional group and the experimental group by the coin tossing method,with 36 cases in each group.The conventional group was treated with sivelestat sodium for injection on the basis of conventional western medical treatment,while the experimental group was treated with modified Dachaihu decoction on the basis of the conventional group.The application effects of the two groups were compared.Results:After treatment,the scores of dizziness,fatigue,chest and abdominal stabbing pain,cold limbs,and high fever in the experimental group were lower than those in the conventional group(P<0.001).After treatment,the levels of interleukin-6,procalcitonin,C-reactive protein,and white blood cell count in the experimental group were lower than those in the conventional group(P<0.05).After treatment,the levels of immunoglobulin A,immunoglobulin G,and immunoglobulin M in the experimental group were lower than those in the conventional group(P<0.05).The duration of mechanical ventilation and length of ICU stay in the experimental group were shorter than those in the conventional group(P<0.001),and the hospitalization cost in the experimental group was lower than that in the conventional group(P<0.001).There was no statistically significant difference in 28-d mortality between the two groups(P>0.05).Conclusion:Modified Dachaihu decoction combined with sivelestat sodium for injection shows significant effects in the treatment of sepsis,and can relieve symptoms,alleviate inflammation,improve immune function,shorten the treatment cycle and reduce the treatment costs.
