超声引导下后路肌间沟臂丛神经阻滞在肩关节镜手术中的应用

Application of Posterior Intermuscular Sulcus Brachial Plexus Nerve Block Guided by Ultrasound in Shoulder Arthroscopic Surgery

摘要目的:分析超声引导下后路肌间沟臂丛神经阻滞在肩关节镜手术中的应用效果。方法:选取2024年9月—2025年2月张家港市中医医院麻醉科接收的肩关节镜手术患者共60例,采用随机数字表法分为参照组与试验组,每组30例。参照组接受全身麻醉,试验组在参照组基础上联合超声引导下后路肌间沟臂丛神经阻滞。比较两组麻醉效果、应激指标、不良反应发生率。结果:试验组麻醉效果优于参照组(P<0.05);麻醉后24 h,试验组去甲肾上腺素、皮质醇水平比参照组低(P<0.001);试验组不良反应总发生率比参照组低(P=0.023)。结论:在肩关节镜手术中实施超声引导下后路肌间沟臂丛神经阻滞的效果显著,可优化麻醉效果,减轻麻醉后应激反应,降低不良反应发生率。 Objective:To analyze the application effect of posterior intermuscular sulcus brachial plexus nerve block guided by ultrasound in shoulder arthroscopic surgery.Methods:A total of sixty patients who underwent shoulder arthroscopic surgery in the department of anesthesiology of Zhangjiagang Hospital of Traditional Chinese Medicine from September 2024 to February 2025 were selected and divided into the reference group and the experimental group by random number table method,with thirty cases in each group.The reference group received general anesthesia,while the experimental group was combined with posterior intermuscular sulcus brachial plexus nerve block guided by ultrasound on the basis of the reference group.The anesthetic effects,stress indicators and incidence of adverse reactions were compared between the two groups.Results:The anesthetic effect of the experimental group was better than that of the reference group(P<0.05).At 24 hours after anesthesia,the levels of norepinephrine and cortisol in the experimental group were lower than those in the reference group(P<0.001).The total incidence of adverse reactions in the experimental group was lower than that in the reference group(P=0.023).Conclusion:The effect of implementing posterior intermuscular sulcus brachial plexus nerve block guided by ultrasound in arthroscopic shoulder surgery is remarkable.It can optimize the anesthesia effect,reduce the stress response after anesthesia,and lower the incidence of adverse reactions.
