摘要
目的:观察经双导丝轨道输尿管镜直视下输尿管扩张术(DG-URSVD)治疗输尿管结石(UC)合并输尿管短段狭窄(USS)的效果。方法:选取2022年5月—2024年12月东莞市凤岗医院收治的UC合并USS患者110例,根据手术方式分为两组,观察组80例采用DG-URSVD治疗,对照组30例采用输尿管支架管置入联合狭窄处扩张及碎石取石术治疗。比较两组手术治疗情况。结果:两组手术取石时间无统计学差异(P>0.05);观察组手术总时间、狭窄扩张时间、住院时间短于对照组(P<0.001),术中出血量少于对照组(P<0.001);观察组狭窄扩张率、结石总清除率高于对照组(P<0.05);观察组并发症总发生率低于对照组(P<0.001);观察组术后再狭窄、再次手术率低于对照组(P<0.001)。结论:DG-URSVD应用于UC合并USS患者中,可以提高手术效率及效果,减少术中出血量及并发症,促进术后恢复,改善预后。
Objective:To observe the effect of ureteral dilation under direct vision via double-wire track ureteroscopy(DG-URSVD)in the treatment of ureteral calculi(UC)combined with short-segment ureteral stricture(USS).Methods:A total of 110 patients with UC combined with USS admitted to Dongguan Fenggang Hospital from May 2022 to December 2024 were selected and divided into two groups according to the surgical methods.80 cases in the observation group were treated with DG-URSVD,and 30 cases in the control group were treated with ureteral stent placement combined with stenosis dilation and lithotripsy and stone removal.The surgical treatment conditions of the two groups were compared.Results:There was no statistically significant difference in the stone removal time between the two groups(P>0.05).The total operation time,stenosis dilation time and hospital stay in the observation group were shorter than those in the control group(P<0.001),and the intraoperative blood loss was less than that in the control group(P<0.001).The stenosis dilation rate and the total stone clearance rate in the observation group were higher than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.001).The rates of restenosis and reoperation after surgery in the observation group were lower than those in the control group(P<0.001).Conclusion:The application of DG-URSVD in patients with UC combined with USS can improve the surgical efficiency and effect,reduce intraoperative blood loss and complications,promote postoperative recovery,and improve prognosis.
