摘要
目的:对比骨质疏松性椎体压缩性骨折(OVCF)患者椎体成形术(PVP)中采用单侧与双侧入路的临床效果。方法:选取2022年1月—2023年12月三明市第二医院收治的OVCF患者196例,按照随机数字表法分成对照组(98例,采取双侧入路PVP治疗),研究组(98例,采取单侧入路PVP治疗)。对两组手术指标、椎体情况及并发症情况进行比较。结果:相较于对照组,研究组骨水泥注入量与透视次数更少(P<0.001),手术操作时间更短(P<0.001)。相较于术前,两组术后3个月Cobb角、伤椎前缘高度、Oswestry功能障碍指数更优(P<0.05);术后3个月,两组Cobb角、伤椎前缘高度、Oswestry功能障碍指数比较,无统计学差异(P>0.05)。研究组骨水泥渗漏发生率低于对照组(P=0.017);两组低血压、感染发生率比较,无统计学差异(P>0.05)。结论:单侧和双侧入路PVP均可改善OVCF患者的Cobb角、伤椎前缘高度及椎体功能,但与双侧入路相比,单侧入路能够减少骨水泥注入量及透视频次,缩短手术操作时间,降低骨水泥渗漏风险。
Objective:To compare the clinical effects of unilateral and bilateral approaches in percutaneous vertebroplasty(PVP)for patients with osteoporotic vertebral compression fractures(OVCF).Methods:A total of one hundred and ninety-eight patients with OVCF admitted to Sanming Second Hospital from January 2022 to December 2023 were selected and divided into the control group(98 cases,treated with bilateral approach PVP)and the study group(98 cases,treated with unilateral approach PVP)according to the random number table method.The surgical indicators,vertebral body conditions and complication conditions of the two groups were compared.Results:Compared with the control group,the study group had less bone cement injection volume and fewer fluoroscopy times(P<0.001),and the operation time was shorter(P<0.001).Compared with that before the operation,the Cobb angle,the height of the anterior edge of the injured vertebrae,and the Oswestry disability index were better in both groups 3 months after the operation(P<0.05).Three months after the operation,there was no statistically significant difference in the Cobb angle,the anterior height of the injured vertebrae,and the Oswestry disability index between the two groups(P>0.05).The incidence of bone cement leakage in the study group was lower than that in the control group(P=0.017).There was no statistically significant difference in the incidence of hypotension and infection between the two groups(P>0.05).Conclusion:Both unilateral and bilateral approaches to PVP can improve the Cobb angle,anterior height of the injured vertebrae and vertebral function in patients with OVCF.However,compared with the bilateral approach,the unilateral approach can reduce the amount of bone cement injection and the frequency of fluoroscopy,shorten the operation time and lower the risk of bone cement leakage.
