摘要
目的评价基于人工智能系统支持的解剖标志定位引流术在小量丘脑出血治疗中的应用价值。方法回顾性分析经人工智能影像诊断系统自主识别非动脉瘤或血管畸形出血,且CTA点征阴性、活动性出血及引流术后再出血低风险的78例小量丘脑出血行引流手术患者的病例资料,其中应用解剖标志定位血肿引流术43例纳入研究组,行立体定向手术机器人辅助引流术35例纳入对照组,对比2组患者手术时间、靶点准确率、血肿清除率、术后90 d治疗效果、预后良好率等。结果研究组与对照组手术时间分别为24.2(15.3,35.6)min及76.2(55.4,87.6)min,有统计学差异(P<0.05);研究组与对照组靶点准确率、血肿清除率、治疗有效率、预后良好率[90.1%比97.1%、84.9%(76.4%,92.8%)比86.7%(81.1%,93.4%)、67.4%比57.1%、72.1%比60%]比较均无统计学差异(P>0.05)。结论基于人工智能系统支持的解剖标志定位引流术治疗丘脑出血操作简便,手术时间短,靶点准确,效果良好。
Objective To evaluate the application value of anatomical landmark localization drainage supported by artificial intelligence(AI)systems in the treatment of minor thalamic hemorrhage.Methods A retrospective analysis was conducted on the clinical data of 78 patients who underwent drainage surgery for minor thalamic hemorrhage,identified by an AI imaging diagnostic system as non-aneurysm or vascular malformation bleeding,with negative computed tomography angiography(CTA)spot sign,low risk of active bleeding and postoperative rebleeding.Among them,43 cases who underwent hematoma drainage surgery with anatomical landmark assisted localization were included in the study group,and 35 cases who underwent robot of stereotactic surgery(ROSS)assisted drainage surgery were included in the control group.The surgical time,target accuracy,hematoma clearance rate,effective treatment and favorable prognosis after 90 days between two groups were compared.Results There was a statistically significant difference(P<0.05)in surgical time between the study group and the control group(24.2(15.3,35.6)min vs 76.2(55.4,87.6)min,P<0.05).There were no statistically significant differences(P>0.05)in target accuracy,hematoma clearance rate,effective treatment and favorable prognosis between the study group and the control group(90.1%vs 97.1%,84.9%(76.4%,92.8%)vs 86.7%(81.1%,93.4%),67.4%vs 57.1%,72.1%vs 60%).Conclusion The anatomical landmark localization drainage surgery supported by AI system for the treatment of thalamic hemorrhage is simple to operate,has a short surgical time,accurate target,and good effect.
