摘要
目的:探讨维持性血液透析(MHD)患者基质Gla蛋白(MGP)、维生素K_(2)水平与血管钙化程度的相关性。方法:选择2020年10月—2023年9月于山西省汾阳医院接受MHD治疗的92例患者为观察组,选择该时间段体检者65例为对照组。观察组给予维生素K_(2)干预2个月后,根据血管钙化评分将其分为无钙化组(n=18)、轻度钙化组(n=33)、中度钙化组(n=26)及重度钙化组(n=15)。比较观察组、对照组MGP及维生素K_(2)水平,比较无钙化组、轻度钙化组、中度钙化组及重度钙化组干预后MGP及维生素K_(2)水平,采用Spearman法分析MGP、维生素K_(2)与MHD患者血管钙化程度的相关性。结果:观察组干预后MGP及维生素K_(2)水平均高于干预前(P<0.001);观察组干预前与干预后MGP及维生素K_(2)水平均低于对照组(P<0.001)。重度钙化组MGP、维生素K_(2)水平低于中度钙化组、轻度钙化组和无钙化组(P<0.05);中度钙化组MGP、维生素K_(2)水平低于轻度钙化组和无钙化组(P<0.05);轻度钙化组MGP、维生素K_(2)水平低于无钙化组(P<0.05)。Spearman相关性分析结果显示,MHD患者MGP、维生素K_(2)水平与血管钙化程度呈负相关(r=-0.451,P=0.024;r=-0.310,P=0.032)。结论:MHD患者MGP水平低于健康人群,给予维生素K_(2)干预后其表达水平升高,MGP、维生素K_(2)水平与血管钙化程度呈负相关。
Objective:To explore the correlation between the levels of matrix Gla protein(MGP)and vitamin K_(2) and the degree of vascular calcification in maintenance hemodialysis(MHD)patients.Methods:A total of ninety-two patients who received MHD treatment at Shanxi Fenyang Hospital from October 2020 to September 2023 were selected as the observation group,and sixtyfive patients who underwent physical examinations during this period were selected as the control group.After the observation group was intervened with vitamin K_(2) for 2 months,it was divided into the non-calcification group(n=18),the mild calcification group(n=33),the moderate calcification group(n=26),and the severe calcification group(n=15)according to the vascular calcification score.The levels of MGP and vitamin K_(2) in the observation group and the control group were compared.The levels of MGP and vitamin K_(2) after intervention were compared among the non-calcification group,the mild calcification group,the moderate calcification group and the severe calcification group.The Spearman method was used to analyze the correlation between MGP,vitamin K_(2) and the degree of vascular calcification in MHD patients.Results:The levels of MGP and vitamin K_(2) in the observation group after the intervention were both higher than those before the intervention(P<0.001).The levels of MGP and vitamin K_(2) in the observation group before and after the intervention were both lower than those in the control group(P<0.001).The levels of MGP and vitamin K_(2) in the severe calcification group were lower than those in the moderate calcification group,the mild calcification group and the non-calcification group(P<0.05).The levels of MGP and vitamin K_(2) in the moderate calcification group were lower than those in the mild calcification group and the non-calcification group(P<0.05).The levels of MGP and vitamin K_(2) in the mild calcification group were lower than those in the non-calcification group(P<0.05).The results of Spearman correlation analysis showed that the levels of MGP and vitamin K_(2) in MHD patients were negatively correlated with the degree of vascular calcification(r=-0.451,P=0.024.r=-0.310,P=0.032).Conclusion:The MGP level in MHD patients is lower than that in healthy people.After intervention with vitamin K_(2),its expression level increases.The levels of MGP and vitamin K_(2) are negatively correlated with the degree of vascular calcification.
