摘要
目的:总结1例外伤性视神经损伤患者行视神经管减压术及腰大池持续引流术的护理个案分析。方法:基于该病例,在护理工作中落实眼科常规护理及专科护理的基础上,联合脑外科行视神经管减压术及腰大池持续引流术围术期的多学科护理,加强眼部病情观察、疼痛护理、各项并发症观察处理、感染预防,腰大池持续引流术后引流管护理、舒适护理等。结果:经过眼科专科联合脑外科多学科个性化、专科的护理,患者获得较好的治疗及护理效果,住院期间未发生出血、感染、脑脊液漏等并发症,疾病恢复良好。结论:眼外伤患者外伤性视神经损伤同期行视神经管减压术及腰大池持续引流术的病例比较少见,除眼科专科护理外,还需MDT多学科团队协作共同制定治疗方案和实施护理措施。围术期存在多方面的病情观察要点,眼科专科护理,如视力、眼压等监测,脑外科专科护理,如视神经管减压术、腰大池持续引流术后的专科护理等。要求眼科护士掌握多学科的治疗及护理知识,这样才能保证患者的各项护理措施正确实施,保证患者的安全和治疗护理效果。
Objective:To summarize the nursing case analysis of a patient with traumatic optic nerve injury who underwent optic canal decompression and continuous lumbar cistern drainage.Methods:Based on this case,in addition to implementing the routine and specialized ophthalmic care,multidisciplinary care was carried out in collaboration with neurosurgery during the perioperative period of optic canal decompression and continuous lumbar cistern drainage.This included enhanced observation of ocular conditions,pain management,monitoring and handling of various complications,infection prevention,postoperative care of the drainage tube,and comfort care.Results:Through the specialized ophthalmic care in conjunction with multidisciplinary care from neurosurgery,the patient achieved good treatment and care outcomes.No complications such as bleeding,infection,or cerebrospinal fluid leakage occurred during the hospital stay,and the patient's recovery was favorable.Conclusion:Cases of traumatic optic nerve injury treated with concurrent optic canal decompression and continuous lumbar cistern drainage are relatively rare.Besides specialized ophthalmic care,multidisciplinary team(MDT)collaboration is necessary to formulate treatment plans and implement care measures.There are multiple key points for condition observation during the perioperative period,including ophthalmic specialized care such as monitoring visual acuity and intraocular pressure,and neurosurgical specialized care such as postoperative care for optic canal decompression and continuous lumbar cistern drainage.It is essential for ophthalmic nurses to master multidisciplinary treatment and care knowledge to ensure the correct implementation of various care measures and guarantee patient safety and treatment outcomes.
