摘要
目的 本研究旨在探讨Ⅰ期肠胃癌患者术后胃肠道功能的发展轨迹,并分析相关影响因素。方法 选择2021年1月—2024年10月在本院进行手术治疗的198例Ⅰ期胃癌/结直肠癌患者作为研究对象,术后对患者进行随访,采用中文版纪念斯隆—凯特林癌症中心(MSKCC)胃肠道功能问卷,于术后1 d、术后1个月、术后3个月、术后6个月对患者的胃肠道功能进行评估,运用潜变量增长混合模型(LGMM)、单因素及多因素分析患者一般资料及胃肠道功能的发展轨迹,并探讨相关影响因素。结果 Ⅰ期肠胃癌患者术后胃肠道功能的发展轨迹可分为持续改善组96例(48.48%)、波动组62例(31.31%)和持续下降组40例(20.21%)。多因素Logistic回归分析结果显示,肿瘤直径、手术时间、术后白细胞计数是Ⅰ期肠胃癌患者术后胃肠道功能的独立危险因素(P<0.05)。结论 Ⅰ期肠胃癌患者术后胃肠道功能存在3条潜在发展轨迹,明确相关危险因素有助于临床制定针对性干预策略,改善患者预后。
Objective To explore the development trajectory of gastrointestinal function in patients with stage I intestinal gastric cancer after surgery,and analyze the relevant influencing factors.Methods A total of 198 patients with stage I gastric cancer/colorectal cancer who underwent surgery in our hospital from January 2021 to October 2024 were selected as the research subjects.Patients were followed up after surgery,and the Chinese version of the Memorial Sloan Kettering Cancer Center(MSKCC)Gastrointestinal Function Questionnaire was used to evaluate their gastrointestinal function at 1 day,1 month,3 months,and 6 months after surgery.The latent variable growth mixed model(LGMM),univariate and multivariate analysis were used to analyze patient general information and the development trajectory of gastrointestinal function,and explore relevant influencing factors.Results The development trajectory of postoperative gastrointestinal function in patients with stage I intestinal gastric cancer can be divided into a continuous improvement group of 96 cases(48.48%),a fluctuation group of 62 cases(31.31%),and a continuous decline group of 40 cases(20.21%).The results of multiple logistic regression analysis showed that tumor diameter,surgical time,and postoperative white blood cell count were independent risk factors for postoperative gastrointestinal function in patients with stage I intestinal gastric cancer(P<0.05).Conclusions There are three potential development trajectories for postoperative gastrointestinal function in patients with stage I gastric cancer.Identifying relevant risk factors can help develop targeted intervention strategies and improve patient prognosis in clinical practice.
