基于二元疾病管理理论的个性化干预在炎症性肠病患儿中的应用效果

摘要 目的:探讨基于二元疾病管理理论的个性化干预在IBD儿童中的应用效果,为完善儿童IBD管理策略提供依据。方法:纳入在本院2022年12月~2024年12月接受治疗的30例IBD患儿及主要照顾者为研究对象,随机分为对照组和观察组各15例。对照组给予常规护理干预,观察组在其基础上,实施基于二元疾病管理理论的个性化干预。干预3个月后,采用McMaster版炎性肠病问卷(IBDQ)、儿童抑郁量表(CDI)、儿童焦虑性情绪障碍筛查表(SCARED)、儿童应对方式量表(CODI)、照顾者负担量表(CBI)、护理满意度问卷进行干预效果评价。结果:干预后,观察组患儿生活质量各维度评分均显著高于对照组(P<0.05),尤以肠道症状及社交功能改善较明显;观察组患儿焦虑、抑郁评分显著低于对照组(P<0.05),积极应对方式评分较对照组显著增高(P<0.05),消极应对方式评分较对照组显著降低(P<0.05);观察组患儿照顾者负担评分显著低于对照组(P<0.05);且观察组患儿及其照顾者护理满意度评分均显著高于对照组(P<0.05)。结论:基于二元疾病管理理论的个性化干预能显著提高IBD患儿的生活质量、改善患儿心理状态及应对方式,减轻照顾者负担,提高护理满意度。 Objective:To investigate the effectiveness of personalized interventions based on dual-disease management theory in children with IBD,providing evidence for refining pediatric IBD management strategies.Methods:Thirty pediatric IBD patients and their primary caregivers treated at our hospital between December 2022 and December 2024 were enrolled and randomly assigned to a control group(n=15)and an observation group(n=15).The control group received routine nursing interventions,while the observation group additionally received personalized interventions based on the dual-disease management theory.After 3 months of intervention,outcomes were assessed using the McMaster Inflammatory Bowel Disease Questionnaire(IBDQ),Children's Depression Inventory(CDI),Screening Checklist for Anxiety and Related Emotional Disorders in Children(SCARED),Children's Coping Orientation Inventory(CODI),Caregiver Burden Inventory(CBI),and Nursing Satisfaction Questionnaire.Results:Post-intervention,the observation group demonstrated significantly higher scores across all quality-of-life dimensions compared to the control group(P<0.05),with particularly notable improvements in bowel symptoms and social functioning.Anxiety and depression scores in the observation group were significantly lower than those in the control group(P<0.05).Positive coping scores were significantly higher(P<0.05),while negative coping scores were significantly lower(P<0.05)compared to the control group.Caregiver burden scores in the observation group were significantly lower than those in the control group(P<0.05).Furthermore,both the children and their caregivers in the observation group reported significantly higher nursing satisfaction scores than the control group(P<0.05).Conclusion:Personalized intervention based on the dual-disease management theory significantly improves the quality of life for children with IBD,enhances their psychological state and coping strategies,reduces caregiver burden,and increases nursing satisfaction.
