一起皮肤炭疽聚集性疫情的分子流行病学特征

摘要 目的对2024年9月河南省洛阳市一起皮肤炭疽聚集性疫情进行病原学检测与分子分型,为炭疽疫情防控提供技术支撑。方法对采集的病例、病畜牛及环境样品进行病原菌分离、鉴定,对分离的5株炭疽芽胞杆菌进行can SNP和MLVA-15分型、聚类分析和全基因组测序(WGS)。结果6份皮肤病灶样本(6/7)、6份环境样本(6/11)和2份病畜材料样本(2/2)检出炭疽芽胞杆菌;5株炭疽芽胞杆菌canSNP分型均为A.Br.001/002型,MLVA-15分型均为MLVA-CHN2型,聚类分析结果显示本次疫情分离的菌株可能与四川省的菌株有一定关联。WGS分析结果显示,5株菌均为cgST-358型,均携带耐药基因Bla1、Bla2、FosB、FosB2、SatA及mphL和主要毒力基因(capA/B/C/D/E)、acpA、acpB、cya、lef和pagA、atxA、inhA。结论宰杀、接触携带炭疽芽孢杆菌强毒株的疫牛为引起本次皮肤炭疽聚集性疫情的主要原因,为今后的炭疽疫情防控及分子溯源提供了参考。 Objective To provide technical support for the prevention and control of anthrax outbreaks through conducting pathogenetic testing and molecular typing a cutaneous anthrax cluster in Luoyang city.Methods Pathogenic bacteria were isolated and identified from the samples collected from cases,sick livestock and environment;Five isolated strains of Bacillus anthracis(B.anthracis)were subjected to canSNP and MLVA-15 typing,cluster analysis and whole genome sequencing(WGS).Results B.anthracis was detected in 6 skin lesion samples,6 environmental samples and 2 samples of sick livestock materials;all 5 strains of B.anthracis were identified as canSNP type A.Br.001/002,and all were identified as MLVA-15 type MLVACHN2.The clustering analysis results showed that strains isolated in this cluster might be related to strains from Sichuan Province.The results of WGS analysis showed that tall five strains were cgST358 type,and all carried the resistance genes Blal,Bla2,FosB,FosB2,SatA,and mphL and the main virulence genes(capA/B/C/D/E),acpA,acpB,cya,lef,and pagA,atxA,and inhA.Conclusions The slaughter and contact with infected cattle carrying strong strains of Bacillus anthracis were the main causes of this skin anthrax outbreak,which provides a reference for the prevention and control of anthrax outbreaks and molecular tracing in the future.
