摘要
目的:探究弥散张量成像(DTI)与小视野体素内不相干运动成像(ZOOMit-IVIM)在不同患病年限的2型糖尿病(T2DM)患者腰椎骨髓微环境改变中的应用价值,以弥补现有诊断方法的不足。方法:本研究为前瞻性研究,纳入2023年7月至2024年10月在鄂尔多斯市中心医院内分泌科住院的83例2型糖尿病患者和44例健康志愿者。所有受检者均接受腰椎T1WI、DTI及ZOOMit-IVIM序列扫描。测量腰1~4椎体的FA值、MD值、D值、D^(*)值及f值,采用独立样本t检验、单因素方差分析和Pearson相关分析等方法,分析正常对照组和2型糖尿病患者腰椎骨髓随患病年限的改变。结果:与健康组相比,患者组腰椎骨髓的FA值、D值和f值显著降低,MD值显著升高(P<0.05);不同患病年限组间的FA、MD、D和f值差异有统计学意义(P<0.05),且FA、D和f值与患病年限呈负相关,MD值与患病年限呈正相关(P<0.05)。结论:磁共振DTI与ZOOMit-IVIM定量成像能够反映2型糖尿病患者腰椎骨髓微环境的改变,与糖尿病性骨质疏松症的发生密切相关,可作为其早期识别的重要工具。然而,本研究样本量较小且未考虑其他代谢因素对骨髓的影响,未来需进一步验证。
Objective:To explore the application value of DTI and ZOOMit-IVIM techniques in evaluating the vertebral bone marrow microenvironment changes in patients with type 2 diabetes mellitus(T2DM)of different disease durations,and to address the limitations of current diagnostic methods.Methods:This prospective study included 83 patients with T2DM and 44 healthy volunteers admitted to the Endocrinology Department of Ordos Central Hospital from July 2023 to October 2024.All subjects underwent lumbar T1WI,DTI,and ZOOMit-IVIM sequence scans.The FA,MD,D,D^(*),and f values of the L1-L4 vertebrae were measured.Independent sample t-tests,one-way ANOVA,and Pearson correlation analysis were used to analyze changes in lumbar bone marrow between the normal control group and T2DM patients with different disease durations.Results:Compared to the healthy group,the FA,D,and f values of lumbar bone marrow in the patient group were significantly decreased,while the MD value was significantly increased(P<0.05).There were significant differences in FA,MD,D,and f values among groups with different disease durations(P<0.05).FA,D,and f values were negatively correlated with disease duration,while MD value was positively correlated with disease duration(P<0.05).Conclusion:Magnetic resonance DTI and ZOOMit-IVIM quantitative imaging can reflect changes of lumbar bone marrow microenvironment in patients with T2DM,which are closely associated with the development of diabetic osteoporosis(DOP)and serve as important tools for its early identification.However,this study had a small sample size and did not consider the impact of other metabolic factors on bone marrow,and further verification is needed in the future.
