超微血管成像技术在评估痰结血瘀型甲状腺结节恶性风险中的应用价值

摘要 目的:探究超微血管成像技术(SMI)在痰结血瘀型甲状腺结节恶性风险中的应用价值。方法:回顾性分析上海市宝山区中西医结合医院2018年12月—2019年11月行甲状腺外科手术并获得病理学确诊的94例101个超声诊断为甲状腺结节且中医辨证为痰结血瘀型患者。运用常规灰阶超声、彩色多普勒超声(CDFI)以及超微血管成像(SMI)观察该类甲状腺结节的灰阶超声特征及血流分布特点。采用χ^(2)检验比较CDFI与SMI显示痰结血瘀型患者的甲状腺结节血流分布模式的差异。结果:SMI的甲状腺结节血流检出率高于CDFI,甲状腺恶性结节SMI的血流分布模式主要以Ⅳ型和Ⅴ型为主,SMI血流分布模式分类对痰结血瘀型患者的甲状腺结节良恶性鉴别有统计学意义。结论:SMI可以提高甲状腺结节中彩色血流信号的检出率,SMI血流分布模式分类对痰结血瘀型患者的甲状腺结节良恶性鉴别有一定的临床价值。 Objective:To explore the application value of Superb Microvascular Imaging(SMI)in assessing the malignant risk of thyroid nodules characterized by phlegm-blood stasis syndrome in traditional Chinese medicine(TCM).Methods:A retrospective analysis was conducted on 94 patients with a total of 101 thyroid nodules who underwent thyroid surgery at Baoshan Integrated Traditional Chinese and Western Medicine hospital in Shanghai between December 2018 and November 2019.All patients were diagnosed with thyroid nodules by ultrasound and classified as having phlegm and blood stasis syndrome according to TCM principles.Conventional grayscale ultrasound,color Doppler flow imaging(CDFI),SMI were used to observe the grayscale ultrasound characteristics and blood flow distribution patterns of these thyroid nodules.The chi-square test was employed to compare the differences in blood flow distribution patterns between CDFI and SMI in patients with thyroid nodules characterized by phlegm-blood stasis syndrome.Results:The detection rate of blood flow in thyroid nodules was significantly higher with SMI compared to CDFI.The blood flow distribution patterns of malignant thyroid nodules on SMI were predominantly type IV and type V.The classification of SMI blood flow distribution patterns demonstrated statistical significance in distinguishing between benign and malignant thyroid nodules in patients with phlegm and blood stasis type.Conclusion:SMI improves the detection rate of color blood flow signals in thyroid nodules.The classification of SMI blood flow distribution patterns provides valuable clinical insights for differentiating between benign and malignant thyroid nodules in patients with phlegm and blood stasis type.
