摘要
目的分析超声引导下神经阻滞结合全身麻醉用于老年下肢骨折手术中的效果。方法选取2020年1月至2024年6月莆田学院附属医院收治的老年下肢骨折患者84例,依据不同麻醉方法分为两组,各42例,对照组采用全身麻醉,观察组则结合超声引导下神经阻滞麻醉。比较两组苏醒时间、拔管时间、不同时间[麻醉前(T1)、插管时(T2)、切皮时(T3)、拔管时(T4)]血流动力学、疼痛评分及镇静评分。结果观察组苏醒及拔管时间短于对照组(P<0.05);观察组T2~T4时的心率及平均动脉压低于对照组(P<0.05);观察组拔管10 min后疼痛评分低于对照组,镇静评分高于对照组(P<0.05);不良反应发生率比较,观察组更低(P<0.05)。结论超声引导下神经阻滞结合全身麻醉用于老年下肢骨折手术,可达到良好的麻醉效果,且对患者血流动力学影响较小,不良反应较少,有效保障手术顺利完成。
Objective To analyze the effect of ultrasound-guided nerve blocks combined with general anesthesia in elderly patients with lower limb fracture surgery.Methods Eighty-four elderly patients with lower limb fractures admitted to the Affiliated Hospital of Putian University from January 2020 to June 2024 were selected and divided into two groups according to their anesthesia methods,with 42 cases in each group.The control group was treated with general anesthesia,and the observation group was treated with ultrasound-guided nerve blocks anesthesia.The recovery time,extubation time hemodynamics,pain score and sedation score at different time[before anesthesia(T1),intubation(T2),skin incision(T3),extubation(T4)]were compared between the two groups.Results The observation group bounced back faster and spent less time under extubation compared to the control group(P<0.05).When it came to heart rate and mean arterial pressure measurements between T2 and T4,the observation group came out on top with lower readings than the control group(P<0.05).Ten minutes after extubation,patients in the observation group reported less pain and showed higher sedation scores than their counterparts in the control group(P<0.05).Furthermore,the observation group experienced fewer adverse reactions across the board(P<0.05).Conclusions For elderly patients undergoing lower limb fracture surgery,combining ultrasound-guided nerve blocks with general anesthesia delivers excellent pain control while keeping vital signs stable.This approach minimizes adverse reactions and ensures the procedure goes off without a hitch,making it a solid choice for this vulnerable population.
