基于生物-心理-社会模型探讨机器人辅助肺部手术患者术后睡眠障碍的影响因素及护理对策

Influencing factors and nursing strategies for postoperative sleep disorders in patients undergoing robot-assisted lung surgery based on the biopsychosocial model

摘要 目的:基于生物-心理-社会模型探讨机器人辅助肺部手术患者术后睡眠障碍的影响因素,并提出相应护理对策。方法:采用前瞻性研究设计,选取2021年1月—2023年12月在同济大学附属上海市肺科医院接受机器人辅助肺部手术的260例患者,以匹兹堡睡眠质量指数(PSQI)评估其术后睡眠质量,将存在睡眠障碍的患者纳入睡眠障碍组,其余患者纳入非睡眠障碍组,分析生物、心理及社会因素对术后睡眠障碍的影响,并通过多元线性回归分析筛选独立影响因素,同时提出针对性护理对策。结果:机器人辅助肺部手术患者术后睡眠障碍的发生率为56.92%。多元线性回归分析结果显示,术前营养状况、术后疼痛评分、术后并发症数量、术后FEV1%pred、术后焦虑及术后抑郁程度均为患者术后睡眠障碍的独立影响因素(P<0.05)。护理干预应从生物、心理和社会三个维度入手,以综合改善患者的术后睡眠质量。结论:机器人辅助肺部手术患者术后睡眠障碍的发生受生物、心理及社会因素共同影响,针对性护理干预有助于改善患者的睡眠质量,提高术后康复效果。 Objective:To explore the influencing factors of postoperative sleep disorders in patients undergoing robot-assisted lung surgery based on the biopsychosocial model,and to propose corresponding nursing countermeasures.Methods:A prospective study design was adopted,involving 260 patients who underwent robot-assisted lung surgery at Shanghai Pulmonary Hospital Affiliated to Tongji University from January 2021 to December 2023.The Pittsburgh sleep quality index(PSQl)was used to assess their postoperative sleep quality.Patients with sleep disorders were assigned to the sleep disorder group,while the others were assigned to the non-sleep disorder group.The impacts of biological,psychological,and social factors on postoperative sleep disorders were analyzed,and independent influencing factors were screened through multiple linear regression analysis.Targeted nursing countermeasures were also proposed.Results:The incidence of postoperative sleep disorders in patients undergoing robot-assisted lung surgery was 56.92%.Multiple linear regression analysis showed that preoperative nutritional status,postoperative pain score,number of postoperative complications.postoperative FEV1%opred,postoperative anxiety,and postoperative depression were independent influencing factors for postoperative sleep disorders(P<0.05).Nursing interventions should use a biopsychosocial framework to comprehensively improve patients?postoperative sleep quality.Conclusion:Postoperative sleep disorders in patients undergoing robot-assisted lung surgery are influenced by biological,psychological,and social factors.Targeted nursing interventions can improve patients'sleep quality,enhance postoperative recovery outcomes.
