摘要
目的:探讨拔毒生肌散联合超声清创术治疗糖尿病足(DFU)Wagner 2、3级创面的临床应用与研究。方法:选取我院在2023年1月至2025年1月收治的81例DFU Wagner分级为2、3级患者为研究对象,采用随机数字表法分为两组。两组患者均予以基础治疗,对照组使用锐器清创治疗,内敷凡士林油纱,并用无菌纱布棉垫加绷带固定;实验组使用超声清创术治疗后加以拔毒生肌散敷撒治疗。对比两组治疗后的临床疗效、治疗的愈合时间和治疗前后VAS评分及炎症指标水平[白细胞介素1β(IL-1β)、白细胞介素6(IL-6)及C反应蛋白(CRP)],以及进行两组的安全性评价。结果:治疗后实验组临床疗效(92.86%)高于对照组(72.80%);治疗后实验组中医证候积分短于对照组;治疗后实验组VAS评分低于对照组;实验组愈合时间短于对照组;治疗后实验组IL-1β、IL-6、CRP均明显低于对照组(均P<0.05);不良反应比较无差异(P>0.05)。结论:拔毒生肌散联合超声清创术治疗DFU Wagner 2、3级创面可缩短患者创面愈合时间,降低其疼痛程度及炎症反应,具有安全、有效的特点。
Objective:To investigate the clinical application and research of Badu Shengji Powder combined with ultrasonic debridement for the treatment of diabetic foot(DFU)Wagner grade 2 and 3 wounds.Methods:81 patients with Wagner grade 2 or 3 DFU admitted to our hospital from January 2023 to January 2025 were selected as the study subjects and divided into two groups using a random number table method.Both groups of patients were given basic treatment.The control group underwent sharp instrument debridement followed by coverage with Vaseline gauze,which was secured with sterile gauze pads and bandages.The experimental group received ultrasonic debridement followed by the topical application of Badu Shengji Powder.The clinical efficacy of the two groups after treatment,the healing time of treatment,the VAS score and the levels of inflammatory indicators[interleukin-1β(IL-1β),interleukin-6(IL-6)and C-reactive protein(CRP)]before and after treatment were compared,and the safety of the two groups was evaluated.Results:The experimental group demonstrated a significantly higher clinical efficacy rate than the control group(92.86%vs.72.80%).Additionally,the experimental group showed significantly lower TCM syndrome scores,lower VAS scores,a shorter wound healing time,and significantly reduced levels of IL-1β,IL-6,and CRP after treatment(all P<0.05).No significant difference was observed in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of Badu Shengji Powder and ultrasonic debridement in the treatment of DFUWagner grade 2 and 3 wounds can shorten the wound healing time of patients,reduce the degree of pain and inflammatory response,demonstrating characteristics of safety and effectiveness.
