摘要
目的评估全程个案化管理联合线上线下延续性康复干预对乳腺癌患者的效果。方法选取江苏大学附属昆山医院2023年1月至12月138例乳腺癌手术患者进行研究,应用信封法随机分为两组(观察组与对照组各69例)。对照组采取常规术后宣教干预,观察组采取全程个案化管理联合线上线下延续性康复干预,对比两组患者的体像水平、应对方式、疲乏情况、心理韧性。结果干预后,两组体像障碍自评量表得分均降低,且观察组低于对照组(P<0.05);干预后,两组患者应对方式中面对评分均较干预前升高,回避和顺应评分均较干预前下降,且组间比较,观察组改善均优于对照组(P<0.05);干预后,两组疲劳严重程度量表得分均降低,且观察组优于对照组(P<0.05);干预后,两组25条目Connor-Davidson心理韧性量表得分均提升,且观察组高于对照组(P<0.05)。结论对乳腺癌患者实施全程个案化管理结合线上线下延续性康复措施,可有效提升患者体像认知,优化应对策略,减轻疲乏症状,并增强心理韧性,适于临床推广。
Objective To evaluate the effect of whole-process case management combined with online and offline continuous rehabilitation intervention in patients with breast cancer.Methods A total of 138 patients diagnosed with breast cancer who underwent surgery in Kunshan Hospital Affiliated to Jiangsu University from January to December 2023 were selected and randomly divided into observation group and control group according to envelope method,with 69 cases respectively.The control group received routine postoperative education intervention,and the observation group received whole-process case management combined with online and offline continuous rehabilitation intervention.The body image level,coping style,fatigue and psychological resilience of the two groups were compared.Results After intervention,the scores of self-rating scale for body image were decreased in both groups,and the index level of observation group was lower than that of control group(P<0.05).After intervention,the face scores in the coping style of the two groups were higher than those before intervention,and the avoidance and compliance scores were lower than those before intervention,and the improvement in the observation group was better than that in the control group(P<0.05).After intervention,the scores of fatigue severity scale in both groups were decreased,and the improvement of index level in observation group was better than that in control group(P<0.05).After intervention,the scores of the 25-item Connor-Davidson resilience scale in both groups were improved,and the improvement of the index level in the observation group was better than that in the control group(P<0.05).Conclusion The implementation of whole-course case management combined with online and offline continuous rehabilitation measures for breast cancer patients can effectively improve their body image cognition,optimize coping strategies,reduce fatigue symptoms,and enhance mental resilience,which is suitable for clinical promotion.
