摘要
经口气管插管(orotracheal intubation, OTI)的妥善固定对保障危重患者的生命安全至关重要,尽管目前已有多种OTI的固定方法,但对于老年口腔结构和形态异常、面颈部烧伤、张口受限、口唇单纯性疱疹病毒感染、口唇/口腔黏膜破溃、俯卧位通气等ICU患者,仍存在固定困难的情况,固定效果不够理想。本文旨在对国内外ICU患者OTI固定困难的原因、固定方法以及固定装置的新发明和新工艺进行系统回顾,以期为临床护理人员提供科学的指导和理论依据。
The secure fixation of an orotracheal intubation(OTI)is critical for ensuring the safety of critically ill patients.Despite the availability of various fixation methods on OTI,significant challenges remain in securing OTIs for specific ICU populations,including elderly patients with abnormal oral anatomy,those with facial and neck burns,patients with limited mouth opening,individuals infected with herpes simplex virus on the lips,those with lip or oral mucosa ulcers,and patients undergoing prone positioning ventilation,and the fixed effect is not ideal.This article aimed to systematically review the causes of difficult OTI fixation,existing fixation methods,and recent innovations and technologies in fixation devices for ICU patients at home and abroad,so as to provide clinical nursing staff with scientific guidance and a theoretical foundation.
