摘要
目的讨论计划、实施、检查、处理(plan do check action,PDCA)循环模式用于小儿肺炎门诊护理管理的效果及对患儿家属不良情绪的影响。方法收集2021年3月~2023年10月来中国医科大学附属盛京医院门诊就诊的112例小儿肺炎患儿的临床资料,进行回顾性分析。根据护理管理方式的不同分为对照组(n=56,行常规护理干预)和观察组(n=56,行常规护理+PDCA循环模式管理)。比较两组干预效果和患儿治疗依从性,观察两组患儿家属干预前后不良情绪和心理负担状况。结果干预后观察组患儿发热消退时间、咳嗽缓解时间、肺啰音消失时间较对照组显著缩短(P<0.05)。观察组输液不良事件发生率显著低于对照组(P<0.05)。干预后观察组患儿治疗依从性明显优于对照组(P<0.05)。干预后观察组汉密尔顿抑郁量表、汉密尔顿焦虑量表及Zarit照顾者负担量表评分均明显低于对照组(P<0.05)。结论PDCA循环模式在小儿肺炎门诊护理中临床效果显著,能明显提升患儿的治疗依从性,改善患儿家属的不良情绪及心理负担,具有临床推广价值。
Objective To study the effect of plan do check action(PDCA)cycle mode on outpatient nursing management of pediatric pneumonia and its impact on the negative emotions of children's families.Methods The clinical dataes of 112 pediatric pneumonia patients who visited Shengjing hospital of china medical university outpatient department from March 2021 to October 2023 were collected,and conduct a retrospective analysis.The patients were divided into observation group(n=56,received routine nursing interventions and PDCA cycle management)and control group(n=56,received routine nursing interventions),which accorded to different nursing management methods.The intervention effects and treatment compliance of two groups of children were compared,the negative emotions and psychological burden of family members of two groups of sick children before and after intervention were observed.[using Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA)] and psychological burden [using Zarit Caregiver Burden Interview(ZBI)].Results After intervention,the shorter fever resolution time,cough relief time and lung rales disappearance time of the observation group were shorter than the control group(P<0.05).The incidence of adverse events during infusion in the observation group was significantly lower than that in the control group(P<0.05).After intervention,the treatment compliance of the observation group was significantly better than that of the control group(P<0.05).Afer intervention,the hamilton depression scale,hamilton anxiety scale,and zarit caregiver burden interview scores of the observation group were significantly lower than those of the control group(P<0.05).Conclusion The PDCA cycle model has significant clinical effects in the management of pediatric pneumonia outpatient nursing,which can significantly improve the treatment compliance of children,improve the negative emotions and psychological burden of their families,and has clinical promotion value.
