摘要
目的研究9条目患者健康问卷抑郁量表(Patient Health Questionnaire-9,PHQ-9)应用在公交车驾驶员抑郁情绪筛查中的信度与效度,并确定筛查的临床界值。方法采用整群抽样,对芜湖市某公交集团1574名公交车驾驶员进行PHQ-9、流调用抑郁自评量表、贝克抑郁量表、抑郁自评量表的调查,同时进行以国际疾病分类第10版抑郁诊断为标准的临床访谈评估,2周后从中随机选取350人完成PHQ-9重测。结果PHQ-9内部一致性和稳定性较好(总分Cronbach'sα=0.861,McDonald'sω=0.968,Spearman-Brown系数=0.933,theta系数=0.963,重测信度=0.826)。主成分分析显示该量表的躯体化-情感因子突出。验证性因素分析显示该量表在公交车驾驶员中的模型拟合度较好(χ^(2/df)=3.310、RMSEA=0.078、RMR=0.006、GFI=0.928、CFI=0.969、NFI=0.968、IFI=0.969)。此外,该量表用于评估公交车驾驶员抑郁情绪的最佳划界分为2分,敏感度为58.33%、特异度为90.47%;受试者工作特征曲线下面积为0.771(95%CI:0.749~0.791)。结论PHQ-9在公交车驾驶员抑郁情绪的评估应用中具有良好的信度与效度,2分为最佳诊断划界分。
Objective To investigate the reliability and validity of the 9-item Patient Health Questionnaire Depression Scale(PHQ-9)application in screening for depressed mood in bus drivers and to determine the clinical cutoff values for screening.Methods Using whole group sampling,1574 bus drivers of a bus group in Wuhu City were surveyed on the PHQ-9,the Center for Epidemiological Survey,Depression Scale,the Beck Depression Inventory,and the Depression Self-Rating Scale,and also assessed with a clinical interview based on the International Classification of Diseases,10th edition,diagnostics of depression,from which 350 were randomly selected to complete a retest of the PHQ-9 after 2 weeks.Results The PHQ-9 has good internal consistency and stability(total score Cronbach'sα=0.861,McDonald's=0.968,Spearman-Brown coefficient=0.933,theta coefficient=0.963,retest reliability=0.826).Principal component analysis revealed a prominent somatization-affection factor for this scale.Validated factor analyses revealed a good model fit for the scale among bus drivers(χ^(2/df)=3.310,RMSEA=0.078,RMR=0.006,GFI=0.928,CFI=0.969,NFI=0.968,IFI=0.969).In addition,the optimal cut-off score for this scale for assessing depressive mood in bus drivers was 2,with a sensitivity of 58.33%and specificity of 90.47%,and the area under the Receiver Operating Characteristic Curve was 0.771(95%CI:0.749 to 0.791).Conclusion The PHQ-9 has good reliability and validity in assessing depressed mood in bus drivers,with a score of 2 as the optimal diagnostic cutoff.
