摘要
蜕膜自然杀伤(dNK)细胞是子宫蜕膜中的主要免疫细胞。dNK细胞发育过程中的功能获得,依赖于NK细胞教育,即dNK细胞所表达的具有高度多态性的杀伤细胞免疫球蛋白样受体(KIR)及CD94/NKG2s,与绒毛外滋养层细胞(EVT)所表达的主要组织相容性复合体(MHC)-Ⅰ类分子结合,这一过程调控了NK细胞的效应功能强弱。dNK细胞通过调控母胎界面免疫耐受来参与妊娠建立与维持,dNK细胞教育缺陷介导的dNK功能异常则是复发性流产(RPL)等不良妊娠结局的潜在病理机制。本文从dNK细胞概述、dNK细胞教育及主要相关分子、教育不良dNK细胞介导RPL的病理机制等方面进行综述,以期为预测和改善dNK细胞相关的RPL患者妊娠结局提供参考。
Decidual natural killer(dNK)cells are the most abundant immune cells in the decidua.The functional acquisition during dNK cell development depends on NK cell education,a process in which the highly polymorphic killer cell immunoglobulinlike receptors(KIRs)and CD94/NKG2s expressed on dNK cells bind to major histocompatibility complex(MHC)classⅠmolecules expressed by extravillous trophoblast(EVT)cells.This interaction regulates the effector functions of NK cells.dNK cells participate in the establishment and maintenance of pregnancy by modulating immune tolerance at the maternal-fetal interface.Defects in dNK cell education leading to dNK dysfunction are potential pathological mechanisms underlying adverse pregnancy outcomes such as recurrent pregnancy loss(RPL).This review summarizes the characteristics of dNK cells,dNK cell education and its key associated molecules,and the pathological mechanisms by which poorly educated dNK cells contribute to RPL,aiming to offer insights for predicting and improving pregnancy outcomes in RPL patients associated with dNK cell abnormalities.
