摘要
目的 探讨正念减压疗法对急性白血病患者疾病不确定感及心理痛苦的干预效果,以期为优化急性白血病患者的护理干预策略提供参考。方法 选取2023年8月-2024年9月南京医科大学第一附属医院收治的160例急性白血病患者,采用随机数字表法分为对照组和研究组,每组80例。对照组患者接受常规疗法干预,研究组患者在对照组基础上联合正念减压疗法干预。比较2组患者干预前后疾病不确定感表、心理痛苦程度、生活质量及护理满意度。结果 干预前,2组患者疾病不确定感量表(MUIS)的疾病不明确性及疾病复杂性2个维度评分及总分比较,差异均无统计学意义(P>0.05);干预后,2组患者MUIS量表各维度评分及总分均低于干预前,且研究组均低于对照组,差异均有统计学意义(P<0.05)。干预前,2组患者心理痛苦量表(PAS)及康纳-戴维森弹性量表(CD-RISC)评分比较,差异均无统计学意义(P>0.05);干预后,2组患者PAS评分均低于干预前且研究组低于对照组,CD-RISC评分均高于干预前且研究组高于对照组,差异均有统计学意义(P<0.05)。干预前,2组患者生活质量综合评定问卷-74(GQOLI-74)的躯体功能、心理功能、社会功能及物质生活状态4个维度评分比较,差异均无统计学意义(P>0.05);干预后,2组患者GQOLI-74各维度评分高于干预前,且研究组均高于对照组,差异均有统计学意义(P<0.05)。研究组患者的护理总满意率为91.25%(73/80),高于对照组的73.75%(59/80),差异有统计学意义(χ^(2)=8.485,P=0.004)。结论 正念减压疗法应用于急性白血病患者的护理干预中,可有效减轻患者的疾病不确定感及心理痛苦,增强患者心理韧性,提高患者生活质量及护理满意度,值得临床推广。
Objective To explore the intervention effects of mindfulness-based stress reduction(MBSR)on disease uncertainty and psychological distress in patients with acute leukemia,with the aim of providing a reference for optimizing nursing intervention strategies for acute leukemia patients.Methods A total of 160 acute leukemia patients treated at the First Affiliated Hospital with Nanjing Medical University from August 2023 to September 2024 were selected and randomly assigned to a control group and a study group,with 80 patients in each group.The control group received conventional therapy,while the study group received conventional therapy combined with MBSR intervention.Managing Uncertainty in Illness Scale(MUIS),psychological distress,quality of life,and degree of satisfaction with nursing were compared between the two groups before and after the intervention.Results Before intervention,there were no statistically significant differences between the two groups in the two subscales(disease ambiguity and disease complexity)and total scores of the MUIS(P>0.05).After intervention,the subscale and total scores of MUIS in both groups were lower than before the intervention,with the study group showing lower scores than the control group,and the differences were statistically significant(P<0.05).Before intervention,there were no statistically significant differences between the two groups in psychological distress,as measured by the Psychache Scale(PAS)and the Connor-Davidson Resilience Scale(CD-RISC;P>0.05).After intervention,PAS scores in both groups were lower than before intervention,with the study group showing lower scores than the control group,and CD-RISC scores were higher than before the intervention,with the study group showing higher scores than the control group;these differences were statistically significant(P<0.05).Before intervention,there were no statistically significant differences between the two groups in the four subscales(physical function,psychological function,social function,and material life)of the Generic Quality Of Life Inventory-74(GQOLI-74;P>0.05).After intervention,the GQOLI-74 scores for each subscale were higher than before the intervention in both groups,with the study group show-ing higher scores than the control group,and the differences were statistically significant(P<0.05).The overall degree of satisfaction with nursing in the study group was 91.25%(73/80),which was significantly higher than the 73.75%(59/80)in the control group(χ^(2)=8.485,P=0.004).Conclusion MBSR applied to nursing interventions for patients with acute leukemia can effectively reduce disease uncertainty and psychological distress,promote psychological resilience,improve quality of life,and increase the degree of satisfaction with nursing,making it worthy of clinical promotion.
