摘要
目的:探讨能谱电子计算机断层扫描(CT)联合多层螺旋CT评估肺腺癌患者化疗效果的价值。方法:选取2021年7月—2023年7月于样本医院接受化疗治疗的86例肺腺癌患者作为研究对象,于化疗4周期后评定化疗效果。化疗前、化疗4周期后进行能谱CT及多层螺旋CT检查,采用受试者工作特征(ROC)曲线分析能谱CT联合多层螺旋CT定量评估肺腺癌患者化疗效果的价值。结果:将部分缓解的46例患者纳入疗效良好组,其余40例患者归入疗效欠佳组。疗效欠佳组病理分期整体高于疗效良好组,且两组患者病理类型比较,差异均有统计学意义(P<0.05)。化疗后,疗效良好组CT值、动脉期碘浓度值(IC)、静脉期IC、动脉期能谱曲线斜率(λHU)、静脉期λHU水平明显低于疗效欠佳组,差异均有统计学意义(P<0.05)。绘制ROC曲线结果显示,能谱CT联合多层螺旋CT评估肺腺癌化疗疗效的曲线下面积(AUC)为0.904,联合评估价值最高。结论:能谱CT联合多层螺旋CT评估肺腺癌患者化疗效果的价值较高,可作为肺腺癌化疗疗效的定量评估手段。
Objective:To explore the value of energy spectrum computed tomography(CT)combined with multislieces helieal CT in evaluating the chemotherapy effect of lung adenocarcinoma patients.Methods:A total of 86 lung adenocarcinoma patients who received chemotherapy at the sample hospital from July 2021 to July 2023 were selected as the study subjects.The effect of chemotherapy was evaluated after 4 cycles of chemotherapy.Energy spectrum CT and multi-slice spiral CT were performed before chemotherapy and after 4 cycles of chemotherapy.The receiver operating characteristic(ROC)curve was used to analyze the value of energy spectrum CT combined with multi-slice spiral CT in quantitatively evaluating the chemotherapy effect of patients with lung adenocarcinoma.Results:A total of 46 patients with partial remission were included in the good efficacy group,and the remaining 40 patients were included in the poor efficacy group.The overall pathological staging of poor efficacy group was higher than the good efficacy group,with statistically significant differences(P<0.05).After chemotherapy,the CT value,arterial and venous iodine concentration(IC),and arterial and venous energy spectrum curve slope(λHU)in the good efficacy group were significantly lower than those in the poor efficacy group(P<0.05).The ROC curve results showed that the area under the curve(AUC)of spectral CT combined with multi-slice spiral CT in evaluating the efficacy of chemotherapy for lung adenocarcinoma was 0.904,and the combined evaluation value was the highest.Conclusion:The combination of energy spectrum CT and multi-slice spiral CT has high value in evaluating the chemotherapy efficacy of lung adenocarcinoma patients,and can be used as a quantitative evaluation method for the chemotherapy efficacy of lung adenocarcinoma.
