PDCA循环结合路径式健康教育在乳腺癌放疗护理中的效果评价 被引量:1

Effect Evaluation of the PDCA Cycle Combined with Pathway-Based Health Education in Nursing Care During Radiotherapy for Breast Cancer

摘要 目的探讨路径式健康教育对乳腺癌放疗患者护理的作用,同时结合PDCA循环评估其对护理质量的影响。方法本研究选取2024年11月至2025年2月郑州大学附属肿瘤医院收治的68例女性乳腺癌放疗患者,依据采用随机数字表法将其分为对照组和研究组,每组各34例。对照组实施常规护理干预;研究组接受常规护理基础上,采用PDCA循环管理模式结合路径式健康教育进行干预。研究通过自我护理能力量表(ESCA)、焦虑自评量表(SAS)、抑郁自评量表(SDS)及乳腺癌生存质量量表(FACT-B)四项评估工具,分别在护理前后对两组患者的自我护理水平、心理状态改善情况、生存质量以及皮肤损害程度[美国肿瘤放射治疗协作组织(RTOG)急性放射损伤分级标准]进行对比分析。结果护理后,两组ESCA各维度评分均升高,且研究组高于对照组,差异有统计学意义(P<0.05)。护理后,两组SDS、SAS评分均较护理前降低,且研究组低于对照组,差异有统计学意义(P<0.05)。护理后,两组FACT-B各维度评分均升高,且研究组高于对照组,差异有统计学意义(P<0.05)。研究组患者护理后皮肤损害程度较对照组轻,差异有统计学意义(P<0.05)。结论对乳腺癌放疗患者实施PDCA循环结合路径式健康教育,可有效提高患者自我管理能力,改善负面情绪,减轻患者皮肤整体损伤程度,提升生活质量。 Objective To examine the effect of pathway-based health education on nursing care for breast cancer patients undergoing radiotherapy and,in combination with the Plan–Do–Check–Act(PDCA)cycle,to evaluate its impact on nursing quality.Methods From November 2024 to February 2025,this study enrolled 68 female breast cancer patients receiving radiotherapy at the Affiliated Cancer Hospital of Zhengzhou University.According to the method of using a random number table,they were divided into a control group(n=34)and a study group(n=34).The control group received routine nursing care.The study group received routine nursing care plus an intervention based on the PDCA cycle combined with pathway-based health education.The Exercise of Self-Care Agency Scale(ESCA),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),and Functional Assessment of Cancer Therapy-Breast(FACT-B)were administered before and after the nursing intervention to compare self-care ability,psychological status,quality of life,and skin injury severity graded by the Radiation Therapy Oncology Group(RTOG)acute radiation morbidity scoring criteria.Results After the nursing intervention,ESCA scores across all dimensions increased in both groups,with higher scores in the study group than in the control group(P<0.05).Post-intervention SAS and SDS scores decreased in both groups,with lower scores in the study group than in the control group(P<0.05).Post-intervention FACT-B scores across all dimensions increased in both groups,with higher scores in the study group than in the control group(P<0.05).Skin injury severity after the intervention was milder in the study group than in the control group(P<0.05).Conclusion Implementing a PDCA cycle combined with pathway-based health education for breast cancer patients undergoing radiotherapy can effectively enhance patients’self-management ability,alleviate negative emotions,reduce overall skin injury,and improve quality of life.
