摘要
目的了解脑卒中后运动性失语患者恢复期疾病体验,为制定针对性的护理干预措施提供参考。方法采用现象学研究,通过目的抽样法选取2022年1月至2022年12月于林州市人民医院神经内科住院治疗的20例恢复期脑卒中后运动性失语患者进行半结构式访谈,运用Colaizzi七步分析法,借助NVivo 12对资料进行分析。结果共提炼出3个主题及8个亚主题,微观系统层面为个体内在体验与情绪应对(语言丧失带来的挫败与无助感、自我认同紊乱与角色适应困难、积极应对与自我调适策略);中观系统层面为家庭与社会支持系统的互动影响(社交回避与苦恼、家庭支持是沟通信心的基石、医护人员的专业支持与沟通辅助);宏观系统层面为社会环境中的困境与期待(公共场所沟通便利措施不足与社会误解、社会支持体系尚待完善)。结论脑卒中后运动性失语患者在恢复期的疾病体验涉及个体、家庭、医疗系统及社会环境多个维度,未来需整合多系统资源,构建综合干预模式,以提升患者的康复成效。
Objective To understand the illness experience of patients with post-stroke motor aphasia during the recovery phase and to provide reference for developing targeted nursing interventions.Methods A phenomenological approach was used.Through purposive sampling,20 inpatients with post-stroke motor aphasia in the recovery phase who were treated in the Department of Neurology of Linzhou People’s Hospital from January to December 2022 were selected for semi-structured interviews.Colaizzi’s seven-step method was applied,and NVivo 12 was used to manage and analyze the data.Results Three themes and eight subthemes were extracted.At the microsystem level,the theme was individual inner experience and emotional coping(frustration and helplessness caused by loss of language;disturbances in self-identity and difficulties with role adaptation;active coping and self-adjustment strategies).At the mesosystem level,the theme was the interactive influence of family and social support systems(social avoidance and distress;family support as the cornerstone of communication confidence;professional support and communication assistance from healthcare staff).At the macrosystem level,the theme was dilemmas and expectations within the social environment(insufficient communication-facilitating measures in public settings and social misunderstanding;the social support system remains to be improved).Conclusion The illness experience of patients with poststroke motor aphasia during the recovery phase involves multiple dimensions,including the individual,family,healthcare system,and social environment.Future work should integrate multi-system resources and build a comprehensive intervention model to improve rehabilitation outcomes.
