摘要
目的探讨基于恐惧-回避模型的心理学指导在多发性骨髓瘤(MM)患者中的应用效果。方法选取厦门大学附属第一医院于2023年2月至2024年6月就诊的114例MM患者,采用随机数字表法将患者分为对照组(n=57,予以常规护理)和模型组(n=57,予以恐惧-回避模型的心理学指导护理),比较两组干预前后心理韧性量表(CD-RISC)、Jalowies应对方式量表及疾病恐惧进展量表的评分差异,以及两组护理满意度差异。结果干预后两组各维度心理韧性评分均比干预前升高(P<0.05),且模型组较对照组更高(P<0.05);干预后,两组各项应对方式评分均比干预前改善(P<0.05),且模型组较对照组改善更明显(P<0.05);治疗后,两组疾病恐惧评分均比干预前降低(P<0.05),且模型组较对照组更低(P<0.05);模型组护理满意度高于对照组(P<0.05)。结论基于恐惧-回避模型的心理学指导用于MM患者,可有效减轻患者疾病恐惧程度,提高患者心理韧性,患者护理满意度较高。
Objective To explore the application effect of psychological guidance based on fear-avoidance model on patients with multiple myeloma(MM).Methods One hundred and fourteen MM patients who were admitted to the First Affiliated Hospital of Xiamen University from February 2023 to June 2024 were selected and divided into the control group(n=57,routine nursing)and the model group(n=57,psychological guidance nursing of fear-avoidance model)by using random number table method.The differences in scores of psychological resilience scale(CD-RISC),Jalowies coping style scale and disease fear progression scale before and after intervention and nursing satisfaction were compared between groups.Results After intervention,the scores of dimensions of psychological resilience in both groups were increased compared to before intervention(P<0.05),and the scores in model group were higher than those in control group(P<0.05).After intervention,the scores of items of coping styles in the two groups were improved compared to before intervention(P<0.05),and the improvements in model group were more obvious than those in control group(P<0.05).After treatment,the scores of disease fear in the two groups were declined compared to before intervention(P<0.05),and the scores were lower in model group(P<0.05).The nursing satisfaction in model group was higher compared to control group(P<0.05).Conclusions Psychological guidance based on fear-avoidance model for MM patients can effectively relieve the degree of disease fear and enhance the psychological resilience of patients,with high nursing satisfaction.
